Promethazine Enhancing the effects of Opioids

The most famous way of using opiates and promethazine together is the codeine- promethazine cough syrup formulation mixed with purple soda or whatever, called "lean". Very popular with black rappers.

A very interesting cutural artifact. Many people of African descent are classed as 'super metabolizers' i.e. a lot more codine or hydrocodone are O-demethylated by the liver so that's an interesting genetic difference.

I mean, nobody is more gutted than me to discover that DJ Screw fatally overdosed on 'lean' and whatever else, I really appreciated the fact that his music was (at the time) totally unique (AFAIK).

Depending on the dosage is the length of the half-life. I take 0.9-1.2mg IR daily. It takes longer than 24hrs for the withdrawals to start. The withdrawals at my dosage last for 3 days causing RLS like methadone, shaking, sweating, rebound heart rate, anger, and insomnia.

8mg every 6hrs. I kept that Rx for 7 months. It was stopped the Zanaflex because I wanted to add Methadone 30mg again to Roxicodone 210mg. I was also taking Clonidine 0.9mg at the same time as the Zanaflex - a double alpha2 diet

Zanaflex at that level has decent withdrawals. It starts like fire going down the back of the neck and moves down the spine. That was how I knew it was 6hrs or it would wake my up during sleep for another dose

Interesting. So the withdrawals end on their own at 3 days or is it due to reintroducing clonidine? I'm surprised an outpatient would have a legit script for over a mg of clonidine.

At 3 days the withdrawals have reached the bottom of the overall curve, some minor effects linger for another 24 hrs.

I have had a Clonidine Rx for 23-24 years. Since it is not scheduled, I refill at day 25 instead of day 30 thus the normal 0.9mg daily increases to 1.2mg for 10 days of the month. My legit Clonidine Rx is 0.9mg daily. Since it is the only medicine I have for my heart issues, refilling earlier is not a concern.

I am in a transition process of tests to determine exactly what BP/Pu meds I need to take. I see the cardiologist in 3 months to review the Chemical Stress Test (Abnormal result), Echo (acceptable), and Holter Monitor (SVT & abnormal high pulse) results. My pulse jumps from normal to 150-170bpm 2 to 3 times per hour all day just from little to no activity. I have always known I have issues with the SNS and pain, stress, anger, and anxiety affect my blood pressure and pulse.

I was taking Toprol ER 100mg, Lisinopril 40mg, Diltiazem 240mg, and Clonidine 0.6mg until November 2024. A 2-3 day stint of Bradycardia (29-34 bpm for 36 hrs) landed me in the hospital for two days. Clonidine was increased to 0.9mg daily and the other 3 were removed. The reserves of those 3 meds are taken as needed depending on what my vitals are or how I am feeling. After keeping a daily medical journal for so long, I normally can feel when my vitals are off.

I wanted to give you a educated and sincere reply to your question, I hope this helps. 2.4mg of Clonidine is the 24hr max dosage but rarely does that happen.
 
On the topic of detoxification, I remember long ago Britlofex (lofexidine) was launched specifically for that purpose. Apparently it's still in use in a low-key manner, but I just recall a friend who was a nurse bringing me all of the very impressive looking marketing giveaways when it was announced.

That’s interesting. Don’t see that much in practice here.
 
@vision conquest Clonidine is one of my favorites not only for opioids, it makes basically everything better. Even heart function. At the hospital I was able to try Clonidine IV and I thought they gave me some kind of opioid because it hits differently than oral.

I saw the doctor yesterday and he removed the promethazine. Long-term use is linked to neuroleptic malignant syndrome (NMS) and it messes with the heart. I was put on it last year to help with a 55 day infusion of Unasyn (ampicillin and sulbactam).

I have not tried Orphenadrine. After reading about it just now, I am going to ask my doctor to try it. Usually a couple months of the year, I take either Flexeril 30mg or Zanaflex 32mg. How do you think Orphenadrine compares to those at the dosages ? ** Edit I just checked, it is not as strong. However it seems useful to me.


Yeh I agree, clonidine is a very underrated compound.
Orphenadrine citrate is a skeletal muscle relaxant but not a particularly strong one, I haven't tried the medication you mentioned, flexeril is cyclobenzaprine if I remember correctly.

I only take orphenadrine when needed and I generally find a dose of 70mg orally to be helpful for anxiety, as well as analgesia and helps with sleep also.

It is probably worth a try, and as you pointed out, it is prescribed by most Dr's far more freely than benzos.
