I chose to ask this here because a google search turned up a discussion that seemed quite informed in the archives on these forums.
Without revealing too much, I will explain my situation.
Several years ago I experienced a near fatal accident that nearly paralyzed me. Since then, I have been on a carousel of various opiate painkillers - which allow me to function on a more normal level with the intense pain I still experience. Currently I am prescribed MS Contin (morphine ER generic 75 MG a day) and Roxicodone (Oxycodone generic 120 MG a day), as well as Valium 6 MG a day for muscle spasms. While this may seem like a huge amount to take in a day for someone without a serious injury, some days when I am more active it still does not even handle my worst pain.
Occasionally, since I am subject to pill counts and drug tests when I have my monthly clinic visit, I will supplement this with occasional heroin use. Since I refuse to inject anything (I would not need to supplement my prescription if I shot my prescription up, but I refuse to do so) - I occasionally smoke heroin so I can make my pill counts.
My question is this; I have read that heroin metabolizes into morphine after about 24 hours, but I have also read that SSID tests detect a specific metabolite for heroin that is present from anywhere between 48 hours to 7 days - depending on usage. While my heroin use is very slight, only 1 or 2 10ths of a gram in a day 3 or 4 days out of a whole month, my daily opiate intake is very significant. I am not entirely sure if my clinic detects for heroin specifically, or just opiate metabolites, along with THC, cocaine, methamphetamine, benzos, and possibly MDMA. I am allowed to test for opiates, but I would like to know if I am at risk if testing for heroin if there is a separation of the surface ID tests they use. I know they use surface ID (strip tests) and not mass spectrometer tests, because the cups i urinate into have the test built into the side, and they just peel away the label and read the results. Is there a way to differentiate between heroin and morphine with a surface ID urine test? If so, given my significant daily opiate intake, how long should I give myself to clean out the heroin?
I know all the tricks about dilution and diuretics - and I always play it safe and drink 2 gallons of water for 2 days before my test and as much as possible before the test (I smoke weed on and off for 10 days after my clinic visit and have never failed), and drink cranberry juice the morning of the test, as well as use the middle of the stream of urine (the metabolites are more concentrated at the beginning, and especially the end of your urine stream) - but should I be worried with 6 days to go? I would love to work out more to raise my metabolism, but as of right now I would probably have to either take more medication and risk a discharge for messing up a pill count, or use more heroin closer to my clinic visit. I do not get tested every visit, because I have been going there for a long time and never tested dirty before - but this week I have smoked heroin 7 days before my appointment. Should I be worried? Can someone explain the specific metabolite that SSID urine screens test for heroin? Am I OK because I am allowed to test for morphine, and heroin after 24 hours will show as morphine, or is this incorrect and they have separate metabolites? I would like to be able to use heroin and do my physical therapy the week before my clinic visit without fear. I look to this site, which appears to be full of knowledgable and informed users for scientific answers. Thank you in advance!
