last night my friend did two doses of heroin they came in little folded up pieces of paper with a stamp on each one saying "black pearl". He snorted the powder, he didnt shoot it. He also drank a bunch of beer. Last night he passed out and we couldnt wake him up but he was still breathing, so we let him sleep. This mornig we still cant wake him up but he is still breathing. Should I take him to the ER or does he just need to sleep this off?

Heroin + a bunch of beer is a very bad thing.
I wouldn't be too worried about it unless the individual shows signs of overdose--slow/heavy/lack of breathing, weak pulse/blood pressure, or bluing of skin.

ok he is breathing slow and he is pale but not especially blue. I cant tell if his pulse is weak or not, but he WILL NOT wake up. My friend and I positioned him so he is sitting up right and he still wont wake up. He is sort of responsive when we shake him and shit, but not extremely.

If you're that concerned about it I'd take him to the hospital. It may not be a big deal, but then again, why risk it? He might be pissed, but it's better off then being dead.
At the very least you need to keep an extremely close eye on him. Don't let him leave your sight, period. If you need to go to the bathroom, do it in a cup or on the floor. Seriously... That second you leave could be all it takes. Be prepared to either call an ambulance or get in the car and take him to the ER at a moments notice. Car keys and phone in hand...
If there's someone else with you, have them call the ER before/while you take him there. It will help them prepare for the incoming patient and will save them time when you arrive.
Just don't be stupid and don't take him because he'd be pissed. I'd rather have someone pissed at me then watch a friend die.

It would probably be a good idea to take a set of vital signs every 5 minutes. You can at least do pulse and respirations, blood pressure is also important, but is usually proportional to pulse.
-pulse: side of the neck or radial(thumb side) of wrist is easiest to feel, count for one minute
-respirations: one respiration is IN--OUT, count for one minute
-temperature: thermometer, you should know how to do this. Do NOT put a glass thermometer in his mouth in case he starts to seizure. If glass is all you have, then don't worry about it.
-blood pressure: if you have a cuff and stethoscope, you probably know how to do it. If for some reason you have a cuff and steth, but don't know how to do it, don't worry about it as you will waste time trying to figure it out(it takes some practice), so concentrate on monitoring the above.
You are not really looking for a specific rate, rather you are looking for a change in the rate. Ex: You take his pulse and it is 70, then 5 minutes later you take it and it is 60, then take him to the hospital.

Threads like this scare me.Do keep us informed about your friend.Maybe it's time for him to stop drinking with his Heroin? Alcohol + any other CNS depressant ( though this was an opiate ) is bad news and this should go without saying. I sincerely hope everything is alright.

we took him to the hospital, he is there right now in critical condition, all we can do now is pray for him. i am very glad we took him, tthank you guys for your help. i will let you know what happens.

I hope for the people involved, and the people that read this, that there is a lesson to be learned. The thing that makes me the most mad is that so few people actually take the time to think about their personal limits, and things like this end up happening. Not only does it fuck up the life of the person who OD's but it also puts a horrible stress on the people around them and it's flat out scary.
It's good that you were there to take him to the hospital, but in the future, my advice is that if there is any question, either take them to the hospital, or call and describe the situation to the doctor, and most of the time they can tell you if you should bring them in. It really scares me the number of times I've been at a party and someone is passed out and everyone there says "they're alright, just sleeping it off." Thats how people die. I hope your friend gets better, and I hope it never happens again. Take care.

take him to hospital
this happened to a close friend, she was passed out for about a day and they finally took her to hospital, her kidneys failed and she had to live hooked up to these machines fore ages and she cannot walk properely, all because she got majorly de hydrated . take him now .

Let's see your friend did a bag of dope drank a bunch of beer & now won't wake up. What the FUCK do you think you should do? Shake the shit out of him, if he wakes up tell him,"walk it off dick," if dude don't move drop him at the er & run like a motherfucker...

(even though this isn't going on anymore) It'd be better to tell them exactly what he did so they could react more specifically.

Dear god, this really is disturbing... he does heroin, can't be woke up, and you ask what to do? Huh? When you nod out on smack you shouldnt be out long... it's people like you that could save lives but instead toss people in back rooms and alow them to die.. sure, your friend was a total idiot and perhaps he deserves to die for his stupidity, but if he is living and you have the chance to save him, you should do what you can...
He may recover, he may not, but if he is unarousable after 30-60 minutes you should take him to the hospital ASAP .. a "heroin nod" can usually be overcome by slapping him around, if he is still non-responsive then he needs medical help ..
Let us know if he dies so we can be sure to remind you that YOU could have saved his life ... (but yes, he did put himself in that position and is truely the one at fault!)

I think it was the beer that truly did him in. I think he would have been fine if he did the smack solo. I understand why he wasn't taken to the hospital right away. People almost go into denial when something like this happens and convince themselves that everything will be ok. I hope he lives, and maybe people will read this and realize what alcohol can do especially when combined with other drugs.
