I am just looking for information/opinions/advice. My adult son has been trying to get enough clean time in order to get the vivitrol shot. Unfortunately he has had a couple of slip ups. Last Wednesday around 3 pm he took 60 mg of oxycontin and then Thursday night around midnight he did it again. Very upset with himself he put to gather a chart of the half life of what he took and yesterday at 10 took 1/4 of a 50 mg naltrexone to see if he would have a precipitous withdrawals. He did, he's doing pretty bad this morning. My question is he has a doctor's appointment at 4 pm tomorrow for the vivitrol shot. Assuming his withdrawals subside is taking the shot tomorrow viable?
 
It is viable in my opinion, 12,5 mg of oral naltrexone is a pretty large dose and he should not get very sick again assuming he has not been using after that. It seems that the shot always has some withdrawal-like symptoms but nothing like precipitated WD when person has a tolerance and active drugs in the system, that is hellish to say the least.

Perhaps taking another 1/4 before the doctors appointment to test the waters again and make sure no severe symptoms emerge would be wise. At least the pill wears of rather quickly if something is going wrong again. But this should not happen again, at least in theory if he has been clean for couple days now and with the current naltrexone experience which has lowered his tolerance for sure.

If he is having that much trouble staying clean he is not a good candidate for the shot. It's best used after a long rehab stay where the patient has had time to get used to sobriety and is in a decent place mentally. Forced sobriety like this is dangerous because the shot offers no help with the cravings depression and other symptoms he will face. This means odds are high he will try to break through the shot with opiates which can lead to OD if he takes a large enough dose. Overall these shots are not the miracle they are touted as. I urge you to consider other options such as maintenance.
