Opioids Pain Management Help

Hey All!

So, I’m currently on pain management regimen with my PCP for my right shoulder. Long story short I got hurt in Afghanistan few years back, and have had multiple right shoulder surgeries due to an IED. My doctor currently has me on 20mg OxyContin 2x daily / 10mg Oxycodone (for breakthrough pain)up to 2.5 tabs daily.

Unfortunately, I just tore my left shoulder rotator cuff two weeks ago. The sports med doc found it on the ultrasound this past Friday. I’ve been in excruciating pain from this new tear, and right now it’s a waiting game for Ortho to see me. My pain is usually managed well, but now with this new injury, my meds don’t even help.

I see my doc this week, and I’m wondering how do I ask for a higher dose to cover this new injury? When I saw him last week for the referral to sports medicine he knew I was suffering greatly, but didn’t offer to up my dose a bit. Now that it’s confirmed, do I ask him to up it? A torn rotator cuff is very painful, and I feel awkward asking for an increase to help my quality of life. What do you all think? Thank you!!

Do not directly ask for another dose you will be labeled a drug seeker and have no chance.

Hammer on points like:

I can no longer function at home
I can no longer work
I skip days without sleeping die to the pain
I’m feeling very discouraged and sad because of the pain
I can’t work and provide for my family because of the pain

Etc etc

Gotta let him come to the conclusion to raise the dose …and if he doesn’t after you give enough of a desperation sob story absolutely nothing will get him the raise the dose do you find another doctor or supplement you own supple illegally (dangerous).

Right that makes sense. It’s a shame that advocating for yourself / asking for an increase after a new injury is a bad thing now. The good thing is I said many of those points to the DO during my exam. My PCP will see those notes this week during our meeting. The sleep / quality of life is where I’m suffering
 
You can ask for an NSAID and a slight increase in your opioid analgesics medications

Asking to increase your Oxycodone might send a minor red flag. Not like you're faking anything but the doctor might be concerned

They also are aware that tolerance occurs over time as well as dependency

It would be wise to tell him your concerns and increased pain levels and enquire if it's possible to add an NSAID as needed for when pain gets severe IN ADDITION to a moderate increase in Oxycodone as needed only when you get "breakthrough pain" Oxy-IR 5/10/20mg tablets or whatever

NSAID - Naproxen
Opioid - Oxycodone
Gabapentinoid - Pregabalin 150mg capsule

Pregabalin or Gabapentin work well with an Opioid for severe pain in addition to the NSAID to reduce inflammation

Also an oral Cannabis jel cap or oil under the tongue for fast & maximum absorption....a THC/CBD split works wonders for pain & inflammation
