Oxy info for pain mgt. w/methadone onboard

Howdy Folks,

I'm a long-term,high-dose methadone clinic 'patient' (20+yrs @ 200mgs/day). Now cancer survivor w/ rheumatoid arthritis and intractable pain. (past 5 yrs). Now @ 350mg/methadone/day + 100mcg/hr Fent gel patch. Not happy w/fent and will be switching soon.

Would love some input on switching to oxycodone (highest mg avail etc). Am seeing my PM doc next week. Fent gel patch (100mcg/hr) is not working. I'm 63 and I've used opiates my whole life (literally) and I've always burned them up real fast. They still work...I just need a shitload onboard.

I'm new to BL and really love what I see. I may or may not have stuff to offer...but I'm very grateful that this forum exists.

I really don't see oxycodone breaking through that dose of methadone. I think that you are going to have to explore non-opioid based medications to treat your breakthrough pain.
 
Thanx for the reply TB. I know, my dose is insane and I've had so many over the yrs. tell me just what you said. SSRI's are out (make me nuts), NSAID's and acetamenaphin(sp?) are out as well (shot liver). I have a great med grade TENS unit that does some great stuff w/ pain and I need to utilize it more. It's fairly complex so I just need to learn how to use it.
I feel certain that a high enough dose of either oxycodone/-morphone will be able to assuage some of my night time agony. I know...sounds like a death wish, but believe me, I know this body and mind pretty well after all these yrs. and plus, I'm not looking for pain elimination, just to click it back a few notches so I can get a bit of sleep back in my life. Presently I "sleep" in a chair and wander around at night like a fucking zombie. I'm also thinking of going up to 400mg of methadone. I'm a bit concerned about toxicity, but if it ain't happened yet?? I knew a guy at Beth Israel MMTP yrs ago that was on 1000mgs/day! I've met a few on 600+, so it's not unheard of. Some of us just burn the shit up.
