Heroin ng/ml cutoff levels on drug test and drinking water

I know we do not give answers for length of time for substances in drug testing. However I am wondering what determines the levels for tests sent to the lab. Is it the amount you take? The length of time? Both? And does the drinking water myth lower the ng/ml levels in your body when tested?
