Nexium with drugs

I've seen a thread in the archive that briefly touches on this, but not to the extent of detail that I want to understand if using nexium with other drugs is a safe procedure. So please don't close this thread.

So Nexium is a drug used to treat GERD and stomach ulcers, which I'm currently on. Now GERD pretty much means no alcohol so I'm turning back to MDMA/Speed/Other stims/Psychedelics as an alternative for socialising and partying every so often.

But as I was reading through this article (I know wikipedia is a bad source) I noticed something that irked me in the drug interactions section:

Esomeprazole is a competitive inhibitor of the enzymes CYP2C19 and CYP2C9, and may therefore interact with drugs that depend on them for metabolism, such as diazepam and warfarin; the concentrations of these drugs may increase if they are used concomitantly with esomeprazole.[9] Conversely, Clopidogrel (Plavix) is an inactive prodrug that partially depends on CYP2C19 for conversion to its active form; inhibition of CYP2C19 blocks the activation of clopidogrel, thus reducing its effects.[10][11]

Drugs that depend on stomach pH for absorption may interact with omeprazole; drugs that depend on an acidic environment (such as ketoconazole or atazanavir) will be poorly absorbed, whereas drugs that are broken down in acidic environments (such as erythromycin) will be absorbed to a greater extent than normal.[9]


How would this effect the absorption of pills/ice/powder/ect. ? Could it be potentially dangerous? Would it be wise to do a tester half dose? Anybody have experience with combining nexium and other drugs?

This concerns me as I also take diazepam irregularly for anxiety/sleep and my Dr did not mention this to me at all. I have actually taken 5mg of valium on nexium without knowing and appeared to be fine. What could go wrong? Would nexium actually act as a potentiator meaning I would need less mdma/speed/ect?

Please help me understand, I want to stay safe.
 
because nexium reduces stomach acid production it will increase absorbtion of drugs like amphetamine.

the valium pretty much won't interact. most drugs used in medicine today aren't prodrugs.
 
I was prescribed omeprazole for a few years, mixed it with pretty much everything; stimulants, opiates, psychedelics you name it. Never noticed any difference except less stomach problems from stimulants.
 
Okay well I tried this combo last night and it is a very, very bad idea.

I did half a point of meth to be on the safe side and everything was fine until I got home and took my dose of nexium before bed (I am on 80mg, so 40 in the morning and 40 before bed). The meth had barely affected me, but I decided not to push it and take more.

Anyway about an hour later I started to get weird muscle twitches. I thought I was having a panic attack so I took some Ativan. 10 minutes later, no better, in fact now I had developed a body tremor all over and was uncontrollably shaking. Deciding I didn't want to add anything else into this cocktail of drugs, I made my way to a hospital as the shaking had gone on for over an hour and was getting worse.

Had an ECG done, which was normal. Blood pressure and heart rate were high. Doctor checked my reflexes and muscles and all was normal. Yet I couldn't stop shaking. The ativan had calmed down my mind so this wasn't a panic attack. The doctor explained that my body had absorbed more of the speed than it should have because of the way the nexium affects the pH of the stomach and that my nervous system was overheated. The ativan had not worked to control the tremors because it's only designed as a mild, short-lasting benzo to help with racing thoughts more than anything else.

He gave me 5mg of valium and within 10 minutes I was fine.

But let me emphasize I had only taken 50mg of meth at the most, maybe less! At no point did I feel 'high'. I don't understand what the heck happened. All I know is that I am terrified of taking anything with nexium again. I have never had body tremors from any panic attacks or any drug-related situations. I have had anxiety on drugs, I have had panic attacks on drugs and I have knowingly over done my doses in the past, but this was one of the worst and most distressing things to ever happen to me.

The night before I had also tried a tiny bit of meth and weed with nexium and it was a brilliant, wonderful night with no problems. I don't understand where I went wrong.
 
Generally speaking, drugs like Nexium (Esmoperazole) change the stomach PH levels. In the case of Diazepam, the valium would be absorbed more quickly and to a greater extent. The end result is 20mg should feel like 30mg, and 30mg should feel more like 45-50mg. (About a 33% increase). Also because of the competitive enzyme interaction with CYP2x the valium will stay in your system longer.

Administering the nexium 20-30 minutes before you take your benzo is a great way to potentiate it, making your supply last longer. You will stay 'high' for longer (if getting high from the benzo is your goal), and the high will be more intense.

This is also true for a number of other drugs, the most notable of which is Codeine.

Take care with interactions and if looking for harm reduction remember to take a lower dose of your benzo (or codeine) if you have previously taken nexium, or other PH altering substances. (Think Tums or Bicarb Soda).

You certainly can drink alcohol while taking Nexium, I do it frequently! :) Mostly beer and wine, with an occasional Scotch.
