need help with pain and suboxone

I need some help as I'm not quite sure what I can do, if anything. I have moderately serious back problems and ended up developing an affinity for opiates while under the care of pain management Dr.'s. I enjoy the euphoria too much. I know someone who convinced me to try Suboxone sub-lingual to get off the pills completely. It's great, now have no desire to even take any pills although in the past year I've worked myself up to now taking 12mgs. I was only taking 8 but the last few days I've had some issues and up'd the dose, so, I've only been on 12 for about 5 days... maybe. Regardless, I am now experiencing significant neck/shoulder/arm pain and will most probably end back up on the table for the third time. The Dr., of course not knowing I'm on sub's only that I'm not taking any pain meds, gave me a script for Dilaudid 2mg. Is there ANY hope of obtaining any relief from my pain? I'm going to assume you're going to say that no, the Dilaudid won't touch you after taking 12mgs Subs and the Subs won't really help that much with the pain. FWIW, shooting is not an option. Any advice?
