Opioids MSContin to Duragesic conversion?

I have looked at several conversion tables and they either dont have good conversions for Duragestic (fent patch), or I dont understand what the numbers mean...they give a calculation by the 2 hour block? The patch lasts 48 to 72 hours, so I am supposed to multiply it? That seems a little iffy.

Can anyone help me out and point me in the right direction for what a an equal dosage might be?

75mg MSContin orally + 75mg Oxycodone orally......but its just the MSContin we are switching for.


In the past I have not found my doctors conversion tables to be accurate.
 
I'm not sure if you used this, but in my experience globalrph's conversions are accurate and I used their fent converter here.

It shows 75mg/day of oral morphine to equal 19.3ug/hour with no reduction for partial cross-tolerance. Multiplying by 72 hours (only a small percent require 48hr dosing) you get 1.3896mg total in 72 hours.

So the patch you'd need would be between the 12.5ug/hour patch and the 25ug/hour patch but slightly closer to the latter.

Since this is scientifically derived conversion, I'm going to close this. People's subjective experiences are unreliable given the amount of variables.

Also, we aren't a fan of (aka don't allow) 'what should I ask my doctor for?' Threads and while this isn't explicitly that, many of the responses will almost inevitably be the same as if it were.
