A friend continues to use coke on a regular basis even though it isn't really getting him high anymore . He has been using for four years or so on and off. Does something in the pathways of the brain change your way of thinking after using for a while. Seems like the first stage of madness.
 
If you mean 'do people become hopelessly and obsessively addicted to cocaine over time' then the answer is yes, they certainly do. As for other brain changes, I don't know the answer. Sounds like your mate is quite addicted though, it's going to take them some serious effort to sort this out.
 
Dopamine. Coke releases it way more than an ordinary situation of happiness and disrupts the usual return pathway in the brain.

It's what makes your friend feel so good or at least did. But keep taking Coke you need more because your brain gets used to that feel good chemical and the ability to produce and use it effectively in the brain reduces.

Eventually you reach a stage that you need the Coke just to even feel normal. It's a long road back once you get to that stage. Best to tell them that all they are doing is destroying their heart, nose and significantly reducing their future health and happiness if they don't stop completely. That's why it's addictive, lots of users are infrequent keep it special occasions and they can enjoy it but do it to often and it's game over.
