A 36 year old neighbour of mine died from a meth induced heart attack last year, so it does happen even if it's relatively rare. We were in the middle of a 4 day heat wave at the time and she'd been binging for days, but no more than her "normal" binges - the additional physical stress was just too much for her body to deal with and she collapsed and died while hanging out washing.
Meth was the only drug which showed up in the toxicology tests which were done as part of her autopsy, but I have no idea how you'd even calculate what dose someone is taking in advance because the purity is so variable. Even at autopsy, you can only calculate how much someone ingested but you're not going to be able to translate that into a certain number of lines or pipes.
Massive organ failure and lethal over-heating seem to be the acute risks from extremely high doses of meth.
