"Major interaction" ? am I going to survive?

Not trying to be dramatic but I ALWAYS look stuff up first online... and didn't this time mostly because my new psychpharm doc didnt say anything about it. "This" being zoloft. I was recently perscribed Zoloft but I have not started it yet...

However last night I went out got quite drunk and for god knows why... decided to take half 1 pill (50mg).


Then I had a 30mg oxycodone , which i insuffulated half in the late morning and half late afternoon... only because I happened to have it for free and I thought it might clear up my hangover a bit. I'm not a regular user of OCs.

threw up a couple times but nothing that stood out as out of the ordinary of a really bad hangover.


Then about 45 minutes ago, due a bunch of reasons.... I decided to take 90mg of vyvanse (not an especially high dose for me... I also don't it all the time, but tolerance is there to stay and it has only ever been smooth and not cracked out like 60mg of adderall would make me..).

I am also perscribed 3mg a day of Klonapin and 750mg of Lithium (tapering off very slowly)

I am terrified of the whole "Seritonin-syndrome" . I don't care how "rare" it says it is... but the fact that vyvanse and Zofloft came up as "RED" and major drug interaction is freaking me out.... im at a cafe and starting to not feel "right". There is always the chance it is just anxiety and the unhealthy day I have had.. but still better safe than sorry... I am trying to get in touch with my doc but it's difficult at the moment.

SO between the Lithium, Oxy, the alcohol the night before and now the hefty amount of Vyvanse.... I am even more concerned.


The only thing keeping me from just rushing to the hospital preemptively is that I only took Zoloft ONCE in my entire life.... and it was half the tab 50mg. If I had been taking it for a while and it was throughly "in my system" I would definitely be paying the ER a visit. However, I am definitely NOT feeling good.

In the meantime while I figure things out, can someone PLEASE give me some insight who is knowledgeable.... Is it possible to basically feel any type of negative effect or interaction, aka any danger from having just taking taking half my prescribed dose ONCE for the first time.?


Because if that is more or less impossible (what I am hoping for...), then there is a very good chance it is just my rather severe anxiety kicking up..... because a night of excessive drinking will create a huge amount of anxiety in general the next day. And then I have a lot of anxiety-ridden things happening to me at the same time , my girlfriend , family shit, business shit etc.... and also of course Vyvanse is a stimulant/amphetamine and although it normally does not, compared to adderall... It for sure can trigger/cause anxiety.

While the possibility of an interaction occurring does exist (and whether or not its serotonin syndrome is impossible for us to tell - there are other kinds of drug interactions besides serotonin syndrome...) we can't tell you for sure. There are plenty of people who are prescribed this combination or similar combinations without any problems.

I can tell you that mixing lithium and alcohol is dangerous, a good friend of mine had a seizure from that combination.

Like I said above, we can't tell for sure its serotonin syndrome or not. A lot of people use "serotonin syndrome" as a catch-all for all drug interactions, which isn't correct. I can tell you the symptoms of serotonin syndrome, however:

-Extreme nausea and vomiting
-Visual distortions/hallucinations
-Headache
-Vertigo
-Tremors
-Fever
-Panic/fear/anxiety/etc

These could be indicative of a hypertensive crisis as well. If you experience any of these symptoms, contact emergency medical services as soon as possible.
 
Thank you that is helpful. The vyvanse has seemed to have "come up" and I am feeling better.... I think mostly because I only took it once and at a low dose that I am fine. But I will monitor myself for any of that... it's tough to decipher what is what at least for right now because of the extreme hang-over ex. throwing up...


And it's interesting you mention that about Lithium and alcohol.... I have (unfortunately) been taking it for quite a while, like 2 years and well, I don't drink too much anymore. But I have certainly done lots of drinking on it, and blacking-out drinking at times. The only interaction I could find about it was basically just added CNS depression.

I will look more into that.
 
^My friend that had that reaction drank a considerable amount, I do that it is recommended that they not be combined in any amounts, however.

what NT said! if things start to get serious, don;t hesitate to call 911!!

make sure to drink plenty of water, and take it easy. lay down and relax...your body doesn't need the extra stress your mind puts out over you worrying if you will die (if that makes sense)
 
^^thnks bro, yeah i will take heed for sure. I'm in the process of getting off of it too so im looking forward to my tapering progression....


Yeah im at a cafe unfortunately or fortunately.... few more people/noise than I would like, but otherwise it is a good chill environment. Just took another klonapin see if that helps. The weird thing is I started feeling 100% again, well more than 100%.... and had a long good talk with my father's friend who is a psychatrist, but then just about 5 minutes ago I started feeling bad again... as in panic feeling, seemingly random like little changes with my vision where things are blurry.

I regret and wouldn't be worrying if I had not taken the Vyvanse but it's well way too late for that. I am just going to like you said, reasure myself that it is just me having some heavy anxiety and making it worse by worrying more over this stuff.

Not as concerned about this part... but also I have never taken an opiate (oxy) and after it has come down THEN taken a stimulant. I also usually don't blow a 30mg oxy just like poof.

It sucks because I know a lot about all the other drugs... especially benzos and vyvanse and lithium too... but really know very little about SSRI's/antidepressants, hence the reason I was holding off on taking them until I talked more with another doctor... but yup, drinking often leads to stupid choices.

I am not ruling it out, but it does seem very unlikely that ONE dose of a single 50mg of zoloft would pose any danger... just since it is one of those drugs (kind of speaking out of my ass here) like Lithium that one pill or dose is completely meaningless and you have to take it for at least a week, much longer with Lithium for example, to actually have your body basically acknowledge/start reacting to it.

And like I said, heavy drinking usually leads to a following day where my anxiety is at least 10x that of my normal level.....


still cant help feeling a little worried. Hope the kpins just calm me down and feel 100% and stay that way...
 
the kpin with definitely help, just make sure not to over-indulge, and stay away from other drugs for the night...

just talked to my dad's friend pharm-psychatrist again and he really assured me that I will be absolutely be OKAY medically. But in general of course all of that combined is not a good idea.

I guess plenty of people are prescribed both stimulants and SSRI's
 
This is the first time you've taken the Zoloft (SSRI) ?...no one can be sure but I think its much much less likely you'd have problems if you've only taken 50mg and never taken any before, its takes at least a few days to get working, the normally quote 2 weeks for subsidence of side effects (so I asumme this means in simple terms its now making the chemical changes in your brain) and 6 week to reach full effect.

Reckon you should be fine, don't over do the Klonapin and definitely ring 911 if you start to show symptoms of serotonin syndrome, although this may be difficult to distinguish from a panic attack but better safe than sorry.
