Lifelong poor appetite/nausea, help me make mirtzapine work again?

So basically, to make a long story short, my appetite has been extremely poor my entire life. Whenever I push myself with eating, I get nauseous. This has been evident since birth. All of my lab tests including blood work and thyroid are normal, I have no vitamin deficiencies and according to my physician, I'm extremely healthy. What is definitely apparent is that my appetite and anxiety are closely linked. Whenever any life stressor hits me, my appetite disappears. I'm also extremely irritable, prone to anger, obsessions, and I have been diagnosed with ADHD and Obsessive Compulsive Disorder. So obviously, something in my brain chemistry is a little off.

I have tried every natural solution possible. Megadosing all water soluble vitamins, various amino acids, probiotics, betaine HCL, 5HTP, Inositol, eating gluten/dairy-free and more. I will say that the vitamins and probiotics, along with living a stress-free life, do help slightly. I was in Greece for a summer with no stress, and just with vitamins and going gluten-free I was able to eat fairly normally. Not a lot compared to the average person, but more than I usually can. But maybe it was just placebo effect? Not sure.

I have been diagnosed with gastroparesis before (slow emptying of stomach), but was tested a second time and it came back negative. Perhaps I get gastroparesis like symptoms when anxiety kicks in, which would explain why I was negative at a later date?

The first time I was prescribed Zoloft, just 50 mg, my entire world changed. Bam, like that, after being on it for about 4 weeks, I was able to eat normally! Eventually I had to increase the dose to 100 mg, and then 150 mg, but after a couple of years on that, I experienced the "lack of motivation" of SSRIs. Later, in 2011, I tried mirtzapine for the first time, 15 mg. Wow, what a miracle drug this was. I was able to eat close to 5,000+ calories a day, which for someone like me, was absolutely insane.

The problem was, the mirtzapine stopped working after about 3 months. The appetite-increasing effects died down. Later on, I was able to get my appetite back while being on 100 mg zoloft, and 2 mg of abilify. The abilify caused that lack of motivation again, and exaggerated my ADHD symptoms. So I decided to get off all of that, and I restarted mirtzapine, about a year after I had last tried it. Bam, the mirtzapine worked again! For about 3 months, then again, the appetite increasing effects died off. So it seems like, I need to give mirtzapine like an entire year break before it works again, or maybe the abilify resensitized my 5-HT2 receptors? I'm not sure. The mirtzapine seems to downregulate the H1 receptors hardcore, it's a very powerful anti-histamine.

I would really like to get mirtzapine working again. I am currently on 60 mg of Cymbalta, which isn't increasing my appetite that much. It's causing tons of sexual side effects, along with lack of motivation. Mirtzapine worked really well for me and didn't cause any side-effects besides the morning hangover, it might have even increased my motivation a little bit. 75 mg of Seroquel does still knock me out, and provide a temporary appetite boost, but I don't want to rely on an anti-psychotic, for fear of side-effects, and also that the effects will go away.

Anyone have any ideas on how to resensitize the receptors so the mirtzapine will work again? I have only tried up to 30 mg before, but 15 seems to work the best for the appetite-inducing effects. Would 45+ mg doses work better? At this point, I feel like my only option is to wait another year, then cycle mirtzapine on one week, and off one week to keep the effects going, maybe while alternating with something else. If anyone has any other solutions, like a low-dose SSRI to combine with mirtzapine, let me know. Cymbalta doesn't seem to be working that well for me. But really, I would rather not be on something that I have to take all the time, if possible.
 
I'm on Cymbalta and it totally kills my appetite. It's known for appetite-reduction.

When was the last time you specifically discussed this with your GP/psychiatrist?
 
I'm actively discussing it currently. Cymbalta increased my appetite on 30 mg, and for the 2nd week or so, then it leveled off. On 60 mg of Cymbalta I can't orgasm at all, and I have no motivation to do anything.

We've decided to give Remeron another shot, but this time I'm going to work up to 45 mg. I've never tried anything higher than 30 mg for an extended period of time. I also noticed that this time when I filled my prescription, I got the Teva brand instead of what I used before, the Aurobindo. I looked online and some people feel that there is a definite difference between the name brand, and the different generic brands. I guess we'll see, I'm starting the Teva brand tonight.

In addition to the Remeron, we're going to augment Buspar for anxiety. I've never been a benzo user so I'm not looking for a huge effect. It might be useful for the 5HT1a agonism compounded with the Remeron. I'm going to start that in a week or so after I've tapered off the cymbalta and I've been on 30 mg Remeron for a week. Ugh, today was my first day down to 30 mg on Cymbalta, already having a ton of anxiety.
 
Had the same problem all my life.. except for I vomit sometimes. It's anxiety it's your nerves. Antidepressants are shit for anxiety. What helps me is oxycontin.
My anxiety vanishes and I go into my own cocoon. Then I eat, eat, eat. Pot doesn't even give me munchies anymore just anxiety..so don't give that a go.

And all those supplements and eating healthy shit. Its placebo. Don't believe me? Keep it up .. you will see you are always going to be the same. It might not hurt it's just not going to fix you.

Now, before I get shit oxcotin, or any opiate is not a long term solution. Sorry I wish I had the answer to what is. You might even turn into a heroin addict. And, form what I hear heroin addict don't eat much because they are more concerned about there fix.

Been on Remeron 30 mg, Teva brand for a few days now. I have tried Remeron 30 mg in the past for over a week before, after being on 15 mg for a long time, and it didn't help with appetite symptoms. Here's where things get interesting.

I'm on Day 3 of buspar, 5 mg in the morning, and 5 mg in early evening. Yesterday, I had an absolutely ravenous appetite after my second dose. I'm talking, I put away over 1500 calories in a single meal, which is normally impossible for me. I ate a decent breakfast this morning, and still had room left after I finished 800+ calories. I normally have no morning appetite.

I'm on my 4th day I believe of 30 mg Cymbalta, coming down for about 4 weeks on 60 mg. My anxiety symptoms have gone down significantly today. The first two days weaning from 60 to 30 on Cymbalta were rough, I was irritable, anxious, and obsessive. Today, I feel extremely activated, almost too energetic. I feel like I'm moving at 200 mph. Could this be the California Rocket Fuel combo (30 mg Cymbalta, 30 mg mirtzapine) kicking in? It feels almost too activating for me. I have also heard that 5HT1a agonism (buspar) can be activating. I am hoping that the extreme appetite came from buspar, because I am planning to wean completely off Cymbalta probably tomorrow or the day after. I could definitely see a low-dose of Cymbalta acting in a synergistic way, as 30 mg temporarily increased my appetite before. Unfortunately, sexual sides from Cymbalta are almost unbearable.

I am pretty stoked right now that my appetite is great, especially since I'm only taking 5 mg of buspar at a time, and some people take 20+ mg. Buspar is giving me no side-effects, and my anxiety is really low today. I am hoping that this effect remains once I ditch Cymbalta.

My next course of action is a trial of Memantine to help with OCD, social anxiety, ADHD, and adderall tolerance. I will create a new thread once I tweak my stack effectively.
 
^^ Sounds like it's good so far! Hope you continue to get good effects from the new med regime.

One thing I was going to suggest about your nausea (and devilinthemoon this could apply to you too??) is that you're possibly gluten-intolerant. I only suggest this because my best friend had the same symptoms for her whole life, had tried all of the available anti-emetics, antacids, proton pump inhibitors etc etc. But one doctor decided to test her for gluten-intolerance and it turns out she is. Since she's been eating gluten-free she hasn't felt nauseated or vomited once!

But yeah, the nausea could also be anxiety-related.

Antidepressants are shit for anxiety. What helps me is oxycontin.
Some anti-depressants are excellent for anxiety. But everyone reacts to a-d's differently.
Case in point is Evisceration's reaction to Cymbalta. It's the total opposite for me!! Zero appetite, increased sex drive and ability to orgasm, increased motivation and energy. Just goes to show that there are no blanket rules when it comes to psychiatric meds.
 
Is Mirtazepine the only tetracyclic AD you've tried? There are a few more, and there's also the very similar tricyclic antidepressants.

Good luck! You're far from alone in having these issues. If you haven't yet, you might get ulcers later in life like I did, so don't think you're dying when/if you throw up blood after a few very stressful days :p
 
I have run a gluten free diet before, and things did improve slightly. I went off of it for a bit because I was so depressed and couldn't get any food down, but I am re-incorporating it now.

I have tried marijuana before. It's hit or miss as far as appetite goes. It increases my anxiety horribly when it wears off. I'm also anti-smoking anything and I don't like to get involved with anything remotely illegal. Marinol could be an option down the road, but getting it prescribed is a nightmare and it's super expensive.

Mirtzapine is the only anti-depressant in it's class that I have tried. I'm not really familiar with anything similar, except Elavil.

UPDATE for tonight. So my appetite was good this morning, but it was just OK or averagish tonight. It seems like I get raging hunger about once or twice per day, and I have to take advantage of it at that time. I got a wave of anxiety today at about 5 PM. Not feeling the extreme energy rush and anxiety-free feeling I had earlier in the day. Tomorrow I am discontinuing Cymbalta, so things may get rough.
 
My appetite is still raging in the mornings, and around 5 PM it starts to normalize. Interesting.

Cymbalta discontinuation has not affected appetite at all, but it is extremely rough. I am on my second day being completely off of it, and I am having extreme mood swings. I'm having intense bouts of irrational anger, extreme irritability...never felt so angry in my entire life. Cymbalta discontinuation is really rough. I'm so happy that my girlfriend is abroad for the month and that I haven't been able to contact her for a couple of days, I for sure would have lashed out on her for no reason. Hopefully I get over it fast.
