Opioids is there anything left?

I could really use some advice from the multitude of opioid experts out there in cyberland... Not to get bogged down on the details, but I have a very real (even the doctors aren't arguing that point), very debilitating pain issue that is essentially ruining my life (doesn't help that I'm severely bipolar as well, and the somatic pain sometimes has a way of exascerbating the psychiatric distress... to the point that I had two suicide attempts last month). I've taken - most recently - dilaudid 16 mg qid and then was switched to fentanyl patches @ 75 mcg. Historically speaking, there isn't an opioid I haven't shot, snorted, smoked, ingested, etc. (pretty much, anyway). I was on meth maintenence (200 mg) for 4 years before kicking it (unpleasant doesn't come close). I still occasionally buy a bundle.

here's the question: I go to the pain management place tomorrow. I want to be able to suggest something (they are at a loss, too), because I just can't take this level of pain anymore. But I can't tihnk of anything I haven't used already at some point. I can't eat; I can't sleep; my life is intolerable.

anybody have any ideas?

Change city, go to a clean, quiet place where you find peace and calm. NYC is...insanity. Pure insanity. I don't even want to begin describing it. I visited it plenty of times, I feel like it drains my energy and leaves me dry, like a dead leave. It's the constant noise...the noise, the smell of garbages vaporizing in the scortching heat fueled by the steaming sewers...the filthy subway, the constant rush...I can't take it for more then 4 days.

Are you mentally stable enough to have the discipline to make this work?

Is there a reason you can't do pain management with something you've used in past?

Do you have a feasible plan?
 
Have you tried oxymorphone?

Maybe if you could tell us what you've taken and whether it worked or what side effects you didn't like we could offer better suggestions?

Well considering oxymorphone (opana) is significantly stronger than hydromorphone (dilaudid) his pain may respond better to it. To the OP if all else fails ask about methadone. Although it is rarely used for pain management I have heard of it being used as a last resort pain management medication. I know a guy who had sickle cell & he went through the whole gambit of pain management drugs also. After the fent patches quit working well for him his doctors put him on 40mg methadone wafers 4 times a day. He said it has been working better than the fent for him.
 
Maybe a combination would be beneficial? Continue the methadone as a base, and something fast acting and potent such as morphine IR or oxymorphone? Just my opinion. Methadone itself has been very beneficial to me, both for my pain and my past addiction issues, though unfortunately in my province, I believe methadone is only available via MMT in my area. (I have nerve damage in my spine, as well as a couple teeth so rotten half of them are missing, and the far back one will break if I clench my jaw anymore from years of smoking harsh chemicals.)

i think i'd probably try to stick with the fentanyl, with ir morphine for breakthrough pain.
a good muscle relaxer added in can help immensely, and allow your other meds to last a bit longer.
carisoprodol's a winner if you can get your hands on it.
i'd say add cannabis in..but you said you're one of the pain clinic crowd, so I assume you're tested regularly.
oxymorphone/hydromorphone can be a godsend, but they're less than ideal for long term pain management.
there's a reason morphine has stood the test of time.
it seems that your condition is chronic and your pain will not dissipate..
so truth be told, if you're abusing your much needed meds, in the end you're unfortunately and most definitely causing yourself more pain.

i am very sorry to hear this, and i hope you find some solace in my advice <3

i think you are looking in the wrong place my friend, you are on some heavy duty opiates and since you still dont receive proper pain relief, it is to my belief that more opiates or any sort of pharmaceutical drug would do you no good. you need to search out some alternative options; someone suggested moving away from NYC. that might help...

are you in so much pain you cant exercise? can you explore acupuncture, or what about marijuana? will your doctor let you smoke weed whilst on opioids? how is your diet, are you getting enough vitamins and minerals to support a healthy body?

it isnt a surprise that your psychological pain exacerbates your physical pain. what do you do form mental strggles like stress and anxiety? are you taking any for your bipolar disorder? are you going to therapy; do you speak with someone on a regular basis about how you feel and ask for help? hows your social life? any family?

do you have any hoobys? maybe you could try therapeutic art, or some yoga, or make some music or what about some relaxing messages??? ooooohhh, that sounds nice %)

have you ever tried bio or neuro feedback?? its cool shit. in the wilderness rehab i was in, there was a neruo feedback dude and he would put little like bobby=pin looking things that clamped onto your ears and would send electrical waves to affect the transmission of your brains Alpha-waves. i think it was called alpha stim?? he would also put some some glases on, and then you would lie down, and the glasses would start to flash different colors. it was really cool and really helpful

If you think meth withdrawal was bad... Be fucking careful with opioids, because the withdrawal from those is a billion times worse. I wish you luck, and dilaudid might have a better effect than around the clock meds, which will raise your tolerance exponentially more. Just try not to abuse them, because opioids are a sneaky addiction... And you don't want that. I wish you luck in finding your pain management that works for you.
