Opioids Interactions between Seroquel and Tramadol

Hi guys,

First of all, I apologize for my bad english, since I am not a native speaker.
My doc prescribed Tramal today and I forgot to mention that I am on 200mg Seroquel. According to my psychologist, they interact with each other and that they even can cause a Serotonin syndrome.
She told me, that i can stop taking Seroquel, as long as my doctor prescribe me the Tramadol.

My question is, how long do i have to wait, before I can take the Tramadol without any risk of causing that syndrom. My last dosage of Seroquel is 18h ago.

MONITOR CLOSELY: The risk of seizures may be increased during coadministration of tramadol with any substance that can reduce the seizure threshold, such as selective serotonin reuptake inhibitors (SSRI antidepressants or anorectics), monoamine oxidase inhibitors, neuroleptic agents, central nervous system stimulants, opioids, tricyclic antidepressants, other tricyclic compounds (e.g., cyclobenzaprine, phenothiazines), carbapenems, cholinergic agents, fluoroquinolones, interferons, chloroquine, mefloquine, lindane, and theophylline. These agents are often individually epileptogenic and may have additive effects when combined. Many of these agents also exhibit CNS- and/or respiratory-depressant effects, which may be enhanced during their concomitant use with tramadol.

MANAGEMENT: Caution is advised if tramadol is administered with any substance that can reduce the seizure threshold, particularly in the elderly and in patients with epilepsy, a history of seizures, or other risk factors for seizures (e.g., head trauma, brain tumor, metabolic disorders, alcohol and drug withdrawal, CNS infections).

Unfortunatly, that's not answering my questions.
My question is, how much time should pass after my last seroquel dosage, that I can take the tramal without any risk.

I called my doctor, he said Seroquel got 6h half life. Therefore he would recommend to wait 2 days before taking the tramadol.
 
If you've waited 18-24hrs you're almost certainly going to be fine. Your doc is certainly looking out for you, but given my practical experience with these drug (as well as many others I've talk or known with in the past), I don't think you really need to wait 48hrs before you can safely take tramadol... 200mg is a good sized dose of Seroquel, but still. If you need to take tramadol before 48hrs, say at 18 or 24hrs, you should almost certainly be fine (of course the more tramadol you take the more dangerous things become).

if you're only taking therapeutic doses of tramadol, you have nothing to worry about, unless you are predisposed to have seizures or have ever experienced serotonin syndrome or like symptoms before. only in that case would I say 48hrs is necessary

I took Seroquel for a little bit to help me sleep in college, and I never had any problems taking tramadol. Although, given what your doc said, I'm not necessarily recommending it, I've certainly taken tramadol with Seroquel being in my system w/o any problems. I've actually never heard of Seroquel+tramadol leading to serotonin syndrome, but I certainly don't know everything... I'd be more concerned with mixing an ssri like lexapro with tramadol, and even then I've rarely heard of a problem (people who have issues that I have heard of seem to be predisposed to have such issues)...
