Are you sure you're not interpreting the dose 5-500 mg wrong? If you got that idea from the pills you extracted it means that a pill contains 5 mg hydrocodone and 500 mg APAP, not that 5 up to 500 is the suggested dosage range. Maybe I'm not giving you enough credit but the way you are writing 5-500 makes me want to ask.
A better way to write this is 5/500.
Also you cannot really casually use the LD50 value to choose a dosage for the very first time you take heavy medication. It is the dose at which 50% of tested subjects die. But at what dose do 25% die, or 10% ? Don't make assumptions like that.
Paranoia or delusions or things of the sort can IIRC be a result of opiate overdose, either acute or chronic.
