Opioids how to relieve constant vomiting in withdrawal?

I am in opioid w/d and I'm doing some hardcore projectile vomiting. The only thing that helps that's not a recreational opioid is loperamide/immodium, but that's a still a mu receptor agonist and I worry that I'm digging myself deeper into a hole - I'm further downregulating my mu receptors, right? Ondansetron and domperiodone aren't doing shit.

Go with the Imodium. It won't "dig you in deeper." At least it hasn't here. It works well. Another thing is, keep in mind that it only lasts for a few days and, by itself, can't kill you or anything. It just sucks. I find Imodium and something for body ache seems to work pretty well - just Ibuprofen is enough but I like to cram like 1600mg into me every 6 hours. That and hydrate... Apple juice, ginger ale, basically anything you CAN get for liquid is a good idea - even coffee and tea are better than nothing.

If you're taking the loperamide at normal dosages, it won't set you back. If it helps, use it. What day are you at in the withdrawal?

Loperamide under the tongue works well for me and has a mild sedating effect. As long as you're not eating handfuls of them, there should be no serious issues.

Other things to try:

Zofran
Ginger root
Cannabis
Benadryl or other antihistamines
Phenergan
Scopolamine

Smoke cannabis and use antihistamines.

promethazine would be ideal..or if you're in the states and have to go otc, give dramamine (sublingual) a go.
when I c/t'd morphine I was literally vomiting every 15 minutes on the dot for 9-10 days straight.
hallucinations set in..not pretty ones either.
smoking weed and forcing myself to sip 1/2 gatorade 1/2 water during and directly after saved me - no kidding.
that and reminding myself that it's all temporary.
good luck dear. i feel for you.

I have never heard of domperidone, so I can't comment on it's effectiveness, but I'm quite surprised ondansetron isn't helping you. They use it to help chemo patients with their nausea. I've also always found it very effective, especially since it comes in those instant dissolving little wafers.

If you have access to phenergan(promethazine) I'd try that. It's definitely my go to antinausea drug.

If you're unable to keep anything down long enough to have a med start to work, you should probably go to the doctor's office or urgent care clinic. You could get a shot of promethazine and one of benadryl(diphenhydramine) and I bet you'd feel a lot better.

If lope is working for you you should take it. As long as you don't take massive doses for months on end you'll be fine. If you just use small amounts to get you through the day I don't think ack you're withdrawal time.

Now most people have already suggested most of these but I'll go ahead and say them anyways:
Phenergan(promethazine) - rx only shot or pill in US
Antihistamines, ie benadryl(diphenhydramine) - otc pill in US with shots being administered at offices
Dramamine - otc pill in the US
Weed
Ginger - ginger snap cookies, ginger ale, crystalized ginger (sold with the spices), etc

Without a prescription about al you can get in north america is dimenhydrinate aka Dramamine aka Gravol, Meclizine (bonamine) and scopolamine (available in transdermal patches OTC in Canada atleast).

With a prescription you can get:

1. Promethazine
2. Metoclopramide
3. Prochlorperazine (trade names compazine and stemetil)
4. Chlorpromazine (trade names Thorazine and Largactil)
5. If you live in Canada you can also get methotrimeprazine aka Levomepromazine (brand name nozinan)
6. Cannabis which can be mixed with either of the above to boost the effects.

The last 3 are really old anti-psychotics that work great for nausea. I have taken everyone besides the promethazine and zofran. The metoclopramide didn't work great for any cause of the nausea i had but it works good for alot of people. Compazine works really well and I've had it via IM injection along with Demerol to stop migraines. Thorazine is much the same as compazine except it's still used alot in treating mental illness. It works really great for opiate withdrawal induced nausea and calms you down as well and will probably help you sleep. Methotrimeprazine is half as potent as Thorazine in it's anti-psychotic effects but it has really strong anti-cholinergic effects so it's far drowsier. It makes seroquel look like Dextroamphetamine by comparison. I have used it during opiate withdrawals lot's of times because it's not only great for stopping nausea and vomiting but it's guaranteed to knock you right out. 15mg's is enough to stop any nausea i had and put me out for a good 10 hours.

I don't know why but the clonidine I take for other symptoms completely eliminates any nausea and puking.

I don't know why it would either. It doesn't effect any of the usual neurotransmitters implicated in causing nausea. It is just a α2 adrenergic agonist which results in the lowering of Norepinephrine and epinephrine in the brain. Maybe it lowers dopamine levels as well as it lowers levels of catecholamine's in the brain and dopamine is one of these. Excess dopamine can cause nausea and vomiting hence why many stimulants as well as opiates trigger nausea and vomiting.. Metoclopramide acts as a dopamine 2 receptor antagonist and so do most anti-psychotics and the D2 antagonism is responsible for most of the anti-emetic effects of these drugs.
