Going to ask my pain management doc for a favor - do you think I'm being reasonable?

Not sure if you put that in as a snippet, but it is true. Endorphines released by your body are 20 times stronger than morphine. Endorphines are released by doing pleasurable things. Consider sex too :)



100% coorect. Before starting using opiates, when I would do something with my gf that was enjoyable or when I won alot of money gambling, I would get this awesome feeling come over me & an after glow. Also, playing sports & exercise releases endorphins.
 
I'd seriously consider a ER/IR med combo like Opana/OxyContin with Dilaudid/Oxycodone for breakthrough pain. I currently get OxyContin 80mg 3x daily with 8 4mg Dilaudid for breakthrough and it works best of the combos I've tried. If you prefer hydromorphone try one of the new ER hydromorphone available though I've heard complaints about their effectiveness. Too bad Palladone is off the market in the US, the Canadian equivalent (HydromorphContin) works great.
 
honestly dude, you are like totally maxed out on your opiate dosages - even if your doctor doesn't think you are. This is because you are getting a large amount of Dilaudid a day and making it way stronger by injecting them all.
I would say your best bet is to try and cut a little out each day until you are at a lower dose, or maybe even using them properly. I don't know, man, you are kinda up the shit crik without a paddle. If you could find suboxone or something a 1-3 week break could really help.

Are you using adjunct therapies like e-stim or accupuncture or EMDR or anything?

Acupuncture literally cut in half the amount of opies I needed because it produced so much better analgesia for at least 5 days to a week after a session. I can't encourage you enough to try this, especially since it sounds like you have a complex pain syndrome that isn't necessarily localized, so getting a traditional 5-point acupuncturist would help so, so much.

It is also someone outside of your pain management doctor who you have confidentiality with, so you don't have to pull any punches with questions about IV use, addiction, etc, and it is someone who if you NEVER lie to, can really, really help you in ways that most people wouldn't think possible.
 
I know I don't need six 8mg dillies a day... On two occasions I thought I might get cut off, so I started tapering. I was down to half my dosage when I found out they were false alarms so I was like "woo, party's not over yet!" and went back to my old ways.

As I said before, I'm no saint - I don't really need these meds for pain, at this point they are about 80% recreational (I do get pain sometimes). The main reasons I take them now are to avoid withdrawls (obviously) and although they don't get me high anymore, they still tend to give me a mild feeling of "nice."

If I make myself skip for 8 or more hours and WD begins to kick in, I do get a nice rush, so there's that.

But at the moment, I don't see the need to quit because they haven't really had a negative impact on my life. When the pain kicks in I have them at the ready, and in between I'm just doing a lot of "pain practise drills."

I do agree that excersize helps a lot. When I used to WD before it was time for my meds, even taking a walk around the block would help a lot (although as you said, the motivation required to get yourself up and out of the house can be staggering).

Sex... Well, what can I say. Sadly opiates do affect the body in the most interesting ways when it comes to this!

you just answered your own question, take a step back and look at this.

Personally, as a chronic pain patient, fuck you.
You're fuckin it up for the rest of us who actually need opiate medications.
