From Methadone to Bupe

Hi guys,

I'm going from 30mg (6mls) of Methadone to the starting dose of Buprenorphine on Monday and am just curious what to expect? I've been on the methadone for about 6 months and am ready to come off it so the Dr has switched me to the Bupe. I think my initial dose will be either 4 or 6mg.

Is there any physical difference between the two in how I will feel on Monday a few hours after my first dose?

My last dose of methadone will be 25mg on Sunday morning then my first dose of Bupe on Monday some time.

Any tips and experiences welcomed!

That doesn't sound like nearly enough time to transition - after 6 months of being on methadone there should be enough built up in your system that you'll experience precipitated withdrawals if you take the bupe the day after dosing the methadone.
 
I should have added that I started on 60mg and have come down from that to the 30mg I'm on daily. The Dr has written on the script that I'm able to dose up to 16mg from the get go if I get withdrawals too bad to handle.

Should I have just chosen to go down on methadone 5mg at a time each two weeks till I get down to 5mg then just drop off from there instead? I really just want to get off it all to be honest. I was put on it for a prescribed opioid dependence and haven't ever done any illicit stuff.

I'm just really over the fact I have to keep going to the pharmacy and Dr all the time.
 
The dose isn't the issue - the issue is that bupe is a twin agonist-antagonist opioid, whereas methadone is a full agonist. This means that if you take bupe before all of the methadone has detached from your opioid receptors (which is going to take longer than 24 hours), you'll be thrown into precipitated withdrawal - basically you'll go straight into the peak of withdrawal as soon as the bupe kicks in, like getting a shot of naloxone, except unlike naloxone, it will last for 16+ hours.

You need to wait until you're in full withdrawal from the methadone before taking the bupe, which from my understanding generally takes 48 - 72 hours.
 
I really recommend discussing this with your doctor in a bit more detail, or doing some reading on the transition from methadone to suboxone, otherwise there's a good chance you're in for a horrible day tomorrow.
 
Yes as crank stated you need to wait until you are at least feeling some w/d symptoms for some time before you want to take the Bupe. Try taking the methadone late Sunday and get the bupe on Tuesday. Get some Valium or benzos of your doc if you dont already take them and the w/ds will be a breeze.

It's tuesday morning now and have asked for a higher dose as I've got up this morning feeling a little jittery and know 4mg won't see me through 24 hours. I've taken 8mg so let's see 24 hours from now.

You'll feel kind of crappy for a week, and not quite right for a week after that. The transition is the tricky bit, lots of people give up in the first week because they assume that's what bupe feels like.

I'm starting to feel ok now that I'm steady on 10-12mg a day but I've noticed that I've been having a constant nagging headache since the transition. I've been trying one or two Panadiene Forte which aren't shaking it but this morning I googled just to make sure I can safely take some Ibuprofen and it came up as no drama so I've taken 2 of them just now to see if it helps and I'll check back in later to say if it has or not.

I am seeing a few people complaining about Bupe headaches, do they last long or will they fade once I'm well and truly past my transitioning period? Cause if they keep up I'm not sure I can cope with them for too much longer to be honest.

Im about to do the swap from methadone to bupe myself for the second time. The first time I went from 60mg of methadone to 16mg of bupe for a week then down to 12mg then soon after 8mg. I find 8mg is the perfect dose, anything more and I don't sleep, im irritable all the time, I get an uncomfortable stimulation throughout my body that is beyond uncomfortable and the annoying thing is that all the AOD workers always denied that it was the bupe and that really pissed me off because I was experiencing it. Once I stayed at 8mg life got much better and it was much better than methadone(that is if your not one of the people that rely on the slight effect methadone provides to stay clean) Bupe gives you back your motivation, your emotions(but not too much that its a shock to the system), to an extent your sex drive will return, obviously not as much as if you were totally substance free but its better than having zero interest in sex like on methadone or heroin.

Im going from 60mg of methadone to however much bupe feels right. This time im going into detox to do the switch to make it a lot easier mentally and physically. The switch is done differently to when I swapped over a few years ago. They used to just make you suffer, now they give you oral morphine and valium(under supervision obviously, the morphine takes a fraction of the time to eliminate than methadone) and after 3 or so days when the methadone is out of your system they stop the morphine, let you withdraw until your clearly uncomfortable then give you bupe in 2mg increments. If you get sick they stop, if it helps your symptoms they will give you 2mg every few hours until you feel better. then sart giving you larger doses like 8mg at a time, and if need be eventually 16-32mg until you feel comfortable enough to go home and after that your done. Its much easier than it once was.

This time im not letting myself become drawn to heroin which will inevitably land me back on methadone or dead. That's why I ended up back on the juice after I managed to switch. I got out of rehab(getting off bupe which I did successfully) and within 6 weeks of leaving I was using between 0.1g-0.4g a day. Well stupid me oneday after a night on the meth decided to IV 2 points instead of the usual 1 point because I thought the extra stimulation would keep me safe. WELL... I had the shot and the next thing I remember was lying on the ground with mum leaning over me doing CPR, and then just hysterical crying and apologizing to mum for being such a horrible son and putting the poor woman through hell over and over. I really have been the most horrible, selfish and absolutely feral son and since that day I ALWAYS keep vials of Narcan in the house, if I have a shot mum wants to know before and after but I have only used heroin about twice since that incident. I cant do it, I cant disappoint her again. She is too good of a mother she deserves so much better!

So good luck to anyone considering doing the switch. Don't listen to the people at the clinic/pharmacy! Everyone goes on about how its a government scam to get people off methadone, that it will never hold you like methadone, all this bullshit that has been made up by someone with very strange ideas. It wont give you any kind of high if you've been tolerant to methadone, but if you want your life to move forward, if you want a job, motivation, the energy to be a better parent etc then bupe is a wonderful drug :)
