What happens often is that one doctor (psychiatrist/neurologist) prescribes you some stuff, and then they send you off and you do or don't go to therapy, but in most cases you don't see the specialist for a long time. I don't know how things work in Germany nowadays, but I remember that when you were in the staatliche Krankenkasse, you couldn't just go to a specialist, you had to consult with your GP first an he had to refer you. The point with the GPs is that in most cases they won't change your prescriptions of any medications previously prescribed by a specialist. But they won't refer you to a specialist unless you ask for it, and give them a reason to. Telling him/her that you want to readjust your medication because you are not feeling good should be enough.
