Drug to welcome death

I have a persistent fear that I will get a terminal illness as my father did and I will have to die a slow agonizing death. But then again it would be worse to die in withdrawals at the same time. So if you realized it was your time and you needed to go peacefully and confidently, what drug or combination of drugs would you take?

^Indeed. It doesn't even sound like extreme mental illness or suicidal ideation is the issue here, so I don't see the express need to provide you with resources actually dedicated to these issues.
