different tolerances for different pills

I was wondering if you can let's say have built a high tolerance for tramadol one day but then try oxy and have no tolerance for it? I just had knee surgery monday and have been prescribed oxycodone but my tolerance has seem to climb fast but I can get vicodin by telling my doc the oxy make me "sick". So if I have a high tolerance for oxy would I have a high tolerance for vicodin or tramadol if I haven't had them in awhile?

Yes. All the "fun" and "abusable" pain pills will either work on mu or delta opiate receptors (mostly mu) and therefore are cross tolerant. Certain things like tramadol, demerol etc will not be completely tolerant because they have other painkilling modes of action.

Use your painkillers as directed - to reduce pain to a manageable level. Your doctor will help you titrate off. Don't escalate your dose, but don't live in agony. Discuss this with your doctor and not us, and perhaps most importantly, don't abuse your pills.

I take them as prescribed to a certain extent. I never go past the max dosage but for tramadol for example I take 2 one hour then 3 the next because I know it works more effectively. I appreciate your input but its a little confusing. Cross tolerant? Are you saying your body will have different tolerances for different medications or does your body have an overall opiate tolerance?

If you are tolerant to Oxycodone you will have a tolerance to Hydrocodone because they are both opiates. That's cross tolerance. Also, Hydrocodone is weaker than Oxycodone.

Thank you very much for clearing that up for me. Its strange because I got off tram to take oxycodone for a few days then took 2 trams today and they seemed to work better..maybe its like a sugar pill effect where I feel like it works

Placebo is certainly a possibility, but tramadol is sort of an odd one. It's pretty distinct from true opiate, so it wouldn't surprise me if it wasn't totally cross tolerant.
