I'm suffering from anxiety,Panic, leading me to overuse prescribed benzo,that,I build tolerance,and have to take it with other "things".
My symptom transforms into daily, rebound, chronic type tension headache,followed by depression.
So a GP prescribes (Paracetamol 500mg+Codeine 30mg) under various different generic brands. So far, when taken 3 tabs in the morning, it gives a positive mood, making me less anxious and tensed.
Now, OTC, there's also Panadeine Extra ( paracetamol 500mg+codeine 15mg). This is half the strength of Rx codeine in combo with Paracetamol.
Strangely, the effect is different.
For a couple of mornings, I took 4tabs only of OTC one, other mornings 2tabs of Rx one.
Still, I notice different effect.
Codeine, is a prodrug, metabolised by CYP4502D6 into morphine (10%).
So codeine is a substrate.
But, when, in the same day, I take two doses of it, Codeine acts as an inhibitor of that isoenzyme, so I derive little effect from it.
Beside that, codeine is also transformed into norcodeine, with little effect on mu receptor, unlike the 10% metabolite being morphine. But, it still exerts painkilling properties.
Many, lack this CYP4502D6, and as far as I know, many are taking codeine, and still not hooked on it, even for an extended period of time, as their bodies can't transform codeine into morphine.
But for those who derive benefit, of codeine's metabolite (morphine) which binds strongly as an agonist at mu receptor, physical and psychological dependence would be higher.
I wanted,in the first place,ask whether anyone feel the different effect of OTC compound codeine ( Paracetamol +codeine 15mg) make a difference effect to the Rx compounded one. But, knowing most population lack this isoenzyme which converts codeine to morphine, I guess, I would get a negative feedback. (Except those whose bodies can convert it to morphine. ).
There was the years, before 1998, that Panadeine forte, had a "tremendous" positive effect to my feeling. Eversince then, somehow, I guess, the manufacturers, derived or used codeine extraction differently. There was no nausea, dizziness, but pure "positive" effect everytime I took 2tabs of Panadeine forte, prior to 1998. ( First time I took it in 96).
Since 1998, it had different effect, with a bit of nausea, with no pure "positive" effect anymore.
( Try taking Lomotil, which is diphenoxylate+atropine, that's how I best describe the opposite effect of Panadeine forte now, and prior to 98. ). Yes, Lomotil will straight away give you dysphoria. Or try taking Kwells ( hyoscine hydrobromide) for motion sickness, that also gives dysphoric feeling. I tried Lomotil ( it is anti diarrhoea) while I was overseas, in a third world country, where manufacturers don't really pay much focus on changing or adding "things" to any meds to prevent it from being abused. Surprising, it has "positive" effect on my mind, as does with the DXM.
After trying overseas,third world country,version of DXM, I then stop taking OTC DXM here in australia. Back in 94, there was a discontinued product for children, Euky Bear Cough Syrup, which contains DXM. At that year, until, 1998, DXM still gives "positive" effect. More to enjoyable, than, hallucination robotic like effect.
Welll, I guess, pharmaceutical have changed eversince.
