BACKGROUND:
I have a very high tolerance to opiates. Been on some type of opiate ('scripts always) off and on for about 4+ years now. In the past month I have been on large doses of Demerol (100-150mgs IM or IV, a few times), Lortab 7.5mg (10+ a day some days) and Dilaudid (4mg IM one time, and 100 2mg pills since June 10 & I have another 30 sitting here to use as needed) for a herniated disc in my back. I will be having major abdominal surgery on August 14th.
QUESTIONS:
1) Is there anyway to quickly reduce my tolerance so I can achieve better pain relief with lower doses after my surgery? I will have a PCA with Demerol in it and also Dilaudid for breakthrough pain when needed.
2) I LOVE the euphoria that I get from a large, fast IV dose of Demerol as it takes the pain away so I will probably be dosing myself a lot in the first few hours after surgery. Will that affect the Dilaudids dosage requirements for breakthrough? If so how long will it be affected for?
I want to get the ost pain relief possible out of the available dosages that I will have available to me and maybe get a little joyous in there too. But I do NOT want to compromise any pain relief! Anyone have any thoughts on my situation? I'd appreciate your opinions...
