Finding effective treatment for TMJ is difficult enough, even when you have stellar insurance. There just isn't much out there: the cause is murky,
and treatment worse. Most of what an oral surgeon can offer you are salvage procedures of a last resort.
 
Your best best is to find a hospital-based "pain clinic," preferably one associated with a university hospital. These
programs are more likely to accept Medicaid. You mentioned that you live in Missouri, the University hospital in
Columbia or St. Louis. These clinics are not pill pushers.
