Opioids Concerta (18 mg) + Tramadol (700 mg) = ???

I was prescribed concetra to treat ADD after suffering for 25 years, but I am addicted to tramadol and will slowly withdraw now that my life problems are supposed to be solved, I know that my tramadol dose is insane, but the concerta dose is very low (18 mg) and was told to double it few days later and I will reduce my tramadol dose to the least level where I don't feel much withdrawals.
so ...
Do you think its safe to combine the two?
Will concerta reduce tramadol withdrawals if I highly decrease it or completely stop it?

Concerta will help with the serotonin, norepinephrine, and dopamine deficits from the Tramadol withdrawal, but it won't help with the actual mu-opioid withdrawals. Mixing the two puts you at great risk of a seizure. Taking just 700mg Tramadol daily is asking for a seizure, let alone mixing it with another stimulant.

It is NOT safe to combine the two. Tramadol + stimulant = lowered seizure threshold. I would switch to something like hydrocodone if you must take the Concerta, or better yet, wait until you're under 100-150mg tramadol before starting the Concerta.

crap! ... I really need concerta for work, but working while feeling the withdrawals isn't a good idea, but also having a seizure isn't something I want to experience.
Its been around 14 hours since the last tramadol pill and I am already feeling the fatigue on my muscles, with flu symptoms but not as bad as before or at least not yet.

I'll try to get hydrocodone, but I don't have high hopes so until then maybe I can just getaway with 200mg tram + 36 concerta? I am a big guy ... almost 2 meters tall / 105 kg
the 18 concerta didn't have any effect ... nothing at all.

Switching from tramadol to another opioid to taper off will only help you avoid withdrawal symptoms from tramadol's opioid effects (which mostly kick in with the M1 metabolite 90+ minutes in). Tramadol's opioid action is much less pronounced than the immediate SNRI mood-lifting effects. You can expect depression and demotivation if you switch, but another opioid to taper down with should help deal with the more physical (flu-like) symptoms of withdrawal. Which effects are worse with a 700 mg/day habit, I couldn't tell you. I found the depressive effects worse than the nearly non-existent physical symptoms when going cold turkey following 300 mg/daily tramadol.

200 mg tramadol (if that's a daily total) and 36 mg Concerta doesn't sound fun to me, but it's not in guaranteed seizure land. If you're talking about 200 mg three times a day, that's different. In any case, I'd start with 18 mg Concerta daily until you take care of the tramadol problem.

Let's roll the "seizure dice". I would NOT combine these two, it would likely render you uncomfortable, anxious, sketched out, trippy, paranoid etc. Forget about sleep, and know that seizures may very well occur. If you want to use the Concerta for wd's I suppose you could if most of the tramadol is out of your system, but it could make certain symptoms worse esp. since it is extended-release methylphenidate. It might help some, but if I were doing this I'd combine the Concerta with a benzo, and employ loperamide as well as NSAIDS for aches. I'm going to emphasize again, if you use Concerta to help with wd symptoms, employ the use of loperamide (Immodium) along with it so you are not shitting and vomitting everywhere.

ok thank you guys ... no seriously I know I was going to mix them both in high doses from tramadol and maybe end up having a seziure while driving or something.
anyway I still don't know what to do, its easy for you to suggest replacing tramadol, but where I am there isn't much options without getting my name into the police database and I don't like that idea!
for now I'll just not take concerta anymore ... and find a suitable solution based on your suggestion and what I have ..
btw what about mixing lyrica with concerta? I think lyrica reduces wd but its damn expensive though I don't have many options!
