I get what you mean now, guess the thread title had me confused when I read it at 5am lol. Have you tried heroin before? I really don't notice much of a different between it and morphine, in the brain it's rapidly converted to 3-MAM and 6-MAM and then into morphine. 6-MAM is a strong opioid by itself but it has a very short half life so most of what you get from heroin is actually morphine, the acetyl groups just make it more fat soluble (basically it's a more efficient delivery method of morphine). Plus if you went and ate a gelcap of heroin it would also be deacetylated by first-pass metabolism and become morphine before reaching the brain, effectively giving you street morphine.
As to why heroin is sold over morphine, I think it just comes down to what users want. Most IV users love the rush more than any other part of the experience, and the unique pharmacology of heroin vs morphine provides that for them.
