Misc Citalopram interactions - advice needed

A friend of mine has been prescribed Citalopram, I'm aware that this is an SSRI and should be taken seriously with regards to Seretonin related interactions.

So here goes, first off. MDMA (or analogs thereof). I'm aware that this is dangerous territory, seretonin syndrome n all. Can anyone provide any information on why pharmacologically its bad? The reality of the situation is that they may well want to do the odd stim at the weekend, probably 5/6-APB since MDMA is pretty crap round here. I don't expect anyone to recommend this of course, but an informed decision is better than nothing.

Second, Ketamine/Methoxetamine etc. As far as i can tell there should be no interactions here but please confirm.

Now heres the biggy, the same friend also suffers from back pain and has been prescribed Tramadol, Co-Dydramol and Amitriptyline. I'm concerned that Tramadol may be a problem because of the seretonin thing, not sure whether the same applies to the other painkillers. Can anyone offer some advise here?

Decent info about these interactions would be appreciated, i've got a reasonably scientific head so i'd rather understand the situation. No need for any simple "don't mix your drugs" type replies.
 
5/6-APB has been speculated to act as a releasing agent and reuptake inhibitor of dopamine, norepinephrine, and serotonin

Your friend will likley have a high risk of developing SS. If your friend is still determined to risk this combination, which is not advised. Ask that he place at least 48hrs between his last dose of Citalopram before use of 5/6-APB. And it would be wise to first experiment with a 1/3 of his normal dose prior to administering a full dose to determine if any risk is present. Keep a thermometer present.

Tramadol + Citalopram Is a well documented combination that leads to SS. Your friend should talk to his doctor about being switched to an opiate that will not also act on serotonin.
