Take our Mental Health and Substance Use Support Survey and make a difference. Once you've completed our quick survey, you'll get entered to win a $100 gift card in our weekly prize draw! This work will help to inform the experiences of Canadians in accessing vital mental health and substance use services. Malatest is conducting this survey for the third year in a row, on behalf of the Canadian Institute for Health Information (CIHI).
The Canadian Institute for Health Information (CIHI) is working with Malatest to understand the barriers you face when trying to access mental health and substance use services in Canada. Your input is essential to identifying the challenges people experience. Please do not complete the survey on behalf of someone else; we want to hear about your own experience. Participation is completely voluntary, meaning you can choose to take part, skip any question, or leave the survey at any time. Your individual responses will be kept anonymous. Your information will be released only when it is combined with information from others. The survey should take approximately 10 minutes of your time.
At the end of the survey, you can enter a prize draw for an opportunity to win one of 70 gift cards valued at $100 each!
