can someone answer my dumbass question? (CWE/APAP/plugging)

I have been lurking for almost 6 months. Let me first say thank you because this place saved my life once. Second I am hoping I am doing this right;-)
My question is this...I have taken way too much tylenol today via hydro es. If I were to do a cwe and plugged it is there still tylenol in my system? As in does plugging pills still cross the liver and am I more protected doing that on top of cwe? Please no slammin me if its stupid:-[

^ How much APAP/Tylenol have you consumed? More than 4g in 24 hours is an overdose and you should seek immediate medical assistance, especially if it is significantly above that level - even if only a little over the limit you should still get your liver function checked out. APAP poisoning can be fatal and time is of the essence - by the stage of actually getting symptoms you have passed the window for effective treatment and are likely to be in the realm of permanent liver/multiorgan failure.

Plugged avoids first-pass metabolism (whereby drugs are shunted past the liver and metabolised before hitting the rest of the circulation in the brain) but once a drug is in the bloodstream it is going to pass through the liver. Plugging does not reduce the harm to the liver from APAP .

CWEs do still contain some APAP no matter how good your technique. 1g can dissolve in 100mls cold water, so 400mls would be 4g which is your maximum daily allowance (you should not consume more than 2g in one go). There is likely to be at least a little leakage through the filter as well. If you have already consumed too much APAP, please don't consume any more, even if you have performed a CWE and please do seek medical help if you have taken an overdose level..

Hi effie. Thank u for ur response. I have indeed passed the daily limit. By a lot actually. How should I go about going to the er without getting myself into trouble? I have on many occasions taken more than I have today but that was without the knowledge of apap I have now. I am not doubting I need to go but can they call the police or anything like that?

I have also taken milk thistle and SAM-e. Do those things make a diff? Should I let them know about those?

Sure, but you should still be heading to a medical professional...

Acetaminophen is indeed absorbed rectally. Plugging a CWE won't protect you any more than drinking it.

^^ In the UK they wouldn't be called for any drugs related ER visit at all, but even if it is different in the US I'd be surprised if they were routinely called for a CWE.. If in doubt just say you are concerned about how much APAP you have consumed.

Your health is far more important than anything; APAP overdose can be fatal and as I said the symptoms arrive too late to be of any help, so please get yourself checked out ASAP. The fact that you have gone over the limit in the past makes me more concerned, not less, as your liver could already not be working at full capacity.

This could be potentially fatal. That is more important than anything else!
