Bill Gates: “First, we’ve got population. The world today has 6.8 billion people. That’s headed up to about nine billion. Now, if we do a really great job on new vaccines, health care, reproductive health services, we could lower that by, perhaps, 10 or 15 percent …”
Access to reproductive health services and education lower birth rates, particularly unwanted pregnancies. Furthermore, increasing the level of health care greatly increases the expectation of children living, further decreasing birth rates as parents do not just keep having as many kids as possible hoping that enough with survive. He is describing how in first world countries, birth rate per capita tends to drop to reach homeostasis (or even eventually a slow reduction as in Europe). This is an example of choosing to interpret a quotation in a way that suits your narrative.
