Opioids Benadryl (oral) + IV oxymorphone - strange experience - need help!

Hey everyone! I've got a question concerning a strange reaction I had taking Benadryl orally about 45 minutes before I took my nighttime IV shot of oxymorphone.

First of all I will say that I have never taken Benadryl. Not even alone. At least I don't think. Maybe when I was younger, but regardless I had no idea of how it would affect me until a few hours ago.

Anyway, I have pretty bad insomnia. I am locating a new psychiatrist, although for the time being I am just trying out different things that will make me drowsy (I stopped using benzos a while ago, so just OTC stuff). So I remembered that my friend had told me that Benadryl might work. He said it completely knocks him out.

After last night of no sleep, I decided to take some Benadryl at around 10:30. And naturally I began to prep my bedtime shot of oxymorphone. Originally, I had no intentions of taking them together on purpose for potentiation effects, etc.. I simply always take a shot of oxymorphone before I go to sleep and wanted to try Benadryl that night.

Of course, however, once I decided to take the Benadryl I was curious as to how it might potentiate my shot and help with the histamine release. I wasn't sure if one needed to add an antihistamine to the shot, although I didn't care if it didn't affect the shot as it was not my original intentions so I simply took it orally.

Well, I was in for a big surprise. I was surprised that I rushed REALLY hard. I did IV a pretty decent sized dose (20mg), although when I have taken a shot this size I never got this high...

Immediately after I took the shot I had a really intense body rush and euphoria. Once I started to nod I was in such a heavenly bliss. However, throughout the experience I got extremely confused. Instead of a simple nod in and out I would nod out and then get extremely confused. I wasn't blacking out but I didn't realize what was going on for brief moments. My hands began to tremble a bit also. It was weird I was really euphoric and confused at the same time. I also had some visuals it was strange (like brighter colors and seeing patterns when my eyes blurred out. My heart was also racing.

I am pretty much back to baseline now. I am just really worried I don't know what happened. I hope somebody knows what could have caused this. Any help would be much appreciated.
 
Oxymorphone, like morphine, is primarily metabolized through glucuronidation which means diphenhydramine as a CYP2D6 inhibitor will have a very limited effect if any. Diphenhydramine is however an H1 antagonist which has been shown to increase the antinociception of narcotic drugs which could explain your greater than usual reaction to your shot.
 
It sounds to me that you had a very strange response to the diphenhydramine, I can attest that depending on the dosage diphenhydramine can cause some very dysphoric effects. How many mgs of diphenhydramine did you take? I have read reports similar to yours, minus the IV shot of oxymorphone, of people abusing diphenhydramine and having dysphoria, confusion, minor closed and open eye visuals, and a feeling of anxiety.
 
Antihistamines like benadryl can definitely potentiate the effects of opioids, benadryl especially with it being a 2D6 inhibitor. Benadryl on it's own can lead to effects like confusion/mild hallucinations, although it's unlikely in small doses. You probably experienced some synergy between the two drugs.
 
I have a horrible reaction to diphen. Even at low doses of 50mg or so, I get weird audio effects, and I get confused and sleepy.
