Advice needed - I'm just baffled

OK, I wont mention anything about who this person is or thier relationship to me.

I will say, they had a couple discs fused in their back, and the operation pretty much fucked them up worse than what it was before.

They are on the following meds - DAILY

1x Durogesic Patch 75ncg - changes every 3 days
1x Lyrica 300mg
2x Tramodol 50mg 4 times a day
2x Panadol Osteo every 6hrs
1x Droutran XR 200mg
1x Diaformin 500mg
1x Nexium 40mg
1x Amloditine 10mg
1x Diamicron 60mg
1x Pretiq 50mg
1x Rami/2 300mg
4x Coloxyl with Sienna
2x Constipation No More
1x Amovane 7-5mg (Sleeping Tablet)

Ummmmmmmmm.... All I can say is WTF! This is like a cocktail to a slow painful death, am I right? (I may be wrong, but all this shit cannot be good for your liver)

Just looking for opinions, because I am thinking the Doctor prescribing this shit needs a bullet.

Next med to be added will be Ketamine (So I am told)

Polypharmacy is a big problem these days, but it's surprisingly common. What makes you think this is a "cocktail to a slow painful death"?

Low-dose ketamine is quite an effective analgesic, and is sometimes used in difficult pain management cases.
 
^ Liver Damage... No? It's just so much for one person to handle... I feel so sorry for this person.

Yes, I know the Ketamine will do them a world of good, I figured out perfect oral dosing of MXE just to try and the person who is using all this stuff (which is NOT me, so don't worry i'm not doing the whole SWIM bull shit), said he felt like he was 30 again! So I am guessing if his pain management Dr. gives a proper smallish dose of K, surely some of this shit can be removed?
 
Just taking more drugs won't necessarily cause liver failure, only certain drugs will do that (e.g. high-dose paracetamol). It would be good to cut the list down a bit, but that may not be easy. It seems like this person also has diabetes and hypertension, and perhaps reflux. The coloxyl and senna is probably for opiate-induced constipation.. Lyrica and Pristiq suggests some neuropathic pain too. This doesn't strike me as an obviously dangerous mix of drugs, but I'm no expert. Tramadol and desvenlafaxine could potentially cause serotonin syndrome, but I assume they're both being prescribed by the same doctor so they should know what are safe doses and combinations.
 
Fair enough, it is fairly long - not the longest I've seen though ;) I hate the idea of "needing" so many drugs too...

If he is worried about it himself, I don't think it would be unreasonable to get someone else to look over the list. Problem is, I don't think most GPs would be good at appropriately cutting down a medication list like that, and most other doctors are too specialised to handle more than their own body system. Geriatricians are great at that sort of thing, as are some pharmacists. Not sure how you go about getting a medicine review with a pharmacist but I know it can be done in some circumstances.

If he isn't worried about it, and his prescribing doctor/s aren't worried about it, it probably isn't a problem :)
 
If he is on all this and being monitored by a decent doctor, there shouldn't be a problem. Tolerance is the bigger issue, and something the doctor should take into account and handle. I'm interested to know more about the ketamine he is going to have added; is this prescription ketamine or obtained otherwise?

Also, why the ranitidine (Reni 2) with the Nexium? Was the Nexium alone not enough? Also, Nexium is a fucking rip off because it is just the patented, active stereoisomer of Acimax. Nexium is esompeprazole, Acimax is the racemic which is just as effective and cheaper because you can get generics of it.
 
yeah i know someone on alot of that stuff, but the lyrica is 3 times a day and throw in Oxys, xanax and liquid morphine and anti b's as well.
And yeah liver damage is one thing. I think they get their liver function tested tho.
Several doctors have gone over the mix and basically just added to it. Severe pain and nerve pain you need it i guess.
 
I guess when you have CHRONIC pain, there isn't much else that can be done.

I have him a certain strain of cannabis from a very good friend, and it helped more than all the drugs put together (although it is funny watching a 59 year old man eating two boxes of pizza shapes and an apple pie when he NEVER eats)... Another reason why Cannabis should be decriminalised for this kind of pain!
 
Cannabis... analgesic, antiemetic, appetite stimulant, mood elevator, sedative. Could there be a more perfect drug to improve quality of life in chronic pain or cancer?

Agreed. Sounds like you're a good friend. :)

Also, PM'd you regarding this but will state something on here too. The Reni with the Acimax is overkill in my opinion, and decreasing stomach acid levels too drastically will alter GI flora and alter his absorption of vitamins and minerals which is not good if his diet is already poor.

I injured my back and was on tramadol 150s (knocked me on my ass) x3, Valium x3, panadol osteo x6, movalis x1 and coloxyl x4 a day. I got off it all soon as I could, but my pain was intense, so it tool six weeks.

I don't doubt SOME of those meds are needed, but I disagree with heaping more drugs into the pile when one doesn't work, which seems to be what's happening here
 
Tramadol is fucking shit - I'd definitely get off that. (it's an SSRI + opioid) The comedown is horrid.

If I were a doc I'd prescribe some low-dose MS Contin and low-dose valium as and when required. That's pretty much it.

And the Lyrica is quite exxy, but not bad.
 
I didnt mind the tramadol What Was It?
The 150 were way too heavy - Id take them, pass out, wake up and it would be time to take them so I would, pass out, and the cycle kept repeating. I had to go back to the doc and ask for 50's. He wanted to give me 100's but I said no. Pusher I tell ya :P

He gave me three scripts for my valium and tramadol so I didnt need to hurry back to see him.
