A lot of great musicians who were intense druggies and a lot of great creativity despite the self-made obstacles.
John Coltrane would be high on my list. In his case he was a great musician and a serious heroin user while playing with Miles Davis but actually got fired by Miles for being too much of a junky (!). After going cleanq, he went on to create his best music ( for eg Blue Train, A Love Supreme).
