Question - is tapering down from a fairly high dose (20-25mg) recommended or should cold turkey be OK?
If you take doses under 25mg for a few days it shouldn't prolong the withdrawals too much and will give a significant amount of relief.
If you take big doses (50mg+) and/or take lope for longer than a few weeks, you will face withdrawals again.
Loperamide is a long acting opiate like methadone, and after long term use, it can cause pretty horrid, drawn out withdrawals from what I've read.
^according to my Nurse's Drug Guide textbook, the half-life is 9-14.5 hours but the therapeutic effects last 24 hours.
When I used lope in w/d or to taper, generally I'd only dose once a day and that way it'd help me most of the day and I could still be regular and not backed up when it wore off shortly before my next dose.
^The only times I used it were either for a very short period of time (usually sandwiched between using morphine or hydrocodone) or as a gradual taper so I've never experienced the withdrawal.
I imagine it would be more drawn out than most short-acting opioids but as you said, considerably more mild. That's just my guess though.
