New opioid approved by FDA in the past few days: tapentadol

No.

Its under drug scheduling review. It wont be out until thats done.
 
FRom what I hear tramadol is not that good. So if its anything like tramadol...it sucks. On wikipedia it stated that its somwhere between tranadol and morphine..hmm prolly woulld be fun for someone with a low opiate tolerance. hmmm I wonder how things will play out for it.

If you've never had tramadol, I dunno if you can asses whether or not tramadol or this new drug are any good. A lot of people use them for pain relief and a lot of people use them for recreational purposes. No need to pass judgment on something you've never tried.
 
I am not really looking forward to the substance, basically just a more potent version of Ultram (tramadol). It is not a direct descendant but its mechanism is much the same. One thing that is always nice is the lack of GI motility and the lack of active metabolites (in this case metabolites at all). We did a thread on it a longwhile ago in Advanced, although I really did not see the point in chiming in onit there.

Opioids with SSRI properties definitely are not my "cup of tea" (no pun intended since there is a current volatile thread on Pod Tea hahaah). I do admit interest in one area, just in terms of clinical science. With the SSRI/opioid combo you run into real problems at very moderate dosages , and so as a significantly more potent substance (as compared to Ultram), I would like to see what the literature says on the oncidence of seizure and/or death due to this mechanism.
 
For many (including me) Tramadol does nothing, but for so many others, it is a great opiate, with sometimes unique antidepressant properties, sometimes THE thing for chronic pain sufferers, sometimes the only opiate that's so easy to get and gives an incredible high still, and that last very long and has quite stimulating properties...

In that case having a stronger derivative of Tramadol prescribed is gonna be interesting for many...
I'm sure many will like it...
 
I actually think that the additional effects tramadol has besides the opiate effects are pretty good. I'm 99% sure I'm CYP2D6 deficient for 3 reasons: DXM lasted about 36 hours for me (and wasn't strongly dissassociating except in huge doses, I could seriously feel 90mg of DXM for 2 days), Codeine was completely uneffective for me, and tramadol produces no real opiate like effects.

That being said, tramadol helps me somewhat with pain, even though it's not opiate effects that are responsible.
 
Have to add: People easily forget that when a substance is cimpared to morphine, in terms of peer reviewed literature on dosing guidelines, etc., is in terms of analgesia and not psychoactivity. Unless the paper is specifically examining psychoactivity or any aspect of it the trial will not be looking at "euphoria," etc.


The substance is rated as "between tramadol and morphine" due to its analgesic profile, not itsrecreational possibilities.


That said, tramadol Is a substance that sits in the extreme, either a person loves it or they hate it. I have never heard of anyone being laissez faire about the substance. While I have never felt a thing from tramadol (it is OTC here and VERY inexpensive), my wife is floored on 100 mgs (she is not a recreational user but has taken it for analgesia).
 
Quasi: You can actually verify your enzymatic condition by paying for a profile with a genetic testing facility. Some people do not even have the enzyme, others have multiples of it. However, your not feeling codeine or tramadol might not have to do with YOUR enzymes but with other factors. Many experienced users say the same thing as you. IF you have been using very heavily, especially after a long period of time, you probably would not feel either one.

The times that I tried codeine and tramadol were at times when I had no appreciable tolerance.

What really seemed to verify it for me was my experiments with Dextromethorphan. I always got a bizarre stimulation and extreme muscle rigidity. I couldn't sleep a wink for 2 days sometimes after I did it. The effects would last well over 24 hours even with rediculously small doses, such as 70mg.

Thanks for the suggestion though about verifying my enzymatic condition. I've thought about it before but figured it would probably be a bit too pricey for me to consider at this point in my life.
 
I really dont think this will really compare to tramadol at all. The modes of action are completely different, and this drug works as a NARI and a mu agonist. I believe this will only distinguish the psychoactive effects. As for pain relief, who knows? It will probably be between tramadol and hydrocodone.
 
effects between tramadol and morphine? anybody else find that extremely vague? That could be compared to, between the effects of darvocet and heroin.

nothing wrong with that. thats just normal i would say. isnt codiene like 1/20 in comparison with hydrocodone? how do the hydros work for ya?

Perhaps, though I don't really feel one way or the other about Tramadol. I have taken it many times and have not experienced any unpleasant effects that would make me hate it, yet subsequently I haven't ever experienced any feelings that would provoke me to say "I love Tramadol", as "I love" morphine or oxycodone. To me, Tramadol is simply okay.
