Lysergamides Microdosing LSD while on drug pressure meds?

Alright, this is a complex question, but I need your help, friends.

Over a year ago I had surgery to replace my aortic valve, undergoing a Ross operation. The guidelines require you to keep your systolic blood pressure under 110 for a year. I am now tapering off most of my drugs but am still taking 2.5 mg of Ramipril every morning and 2.5 mg of Bisoprolol. I’ll completely get off Ramipril in a couple weeks.

This weekend I was thinking of micro dosing a small amount of LSD, maybe 30-40 mcg. At this dose, do you foresee any issues?

My cardiologist basically told me I now have the same risk profile as a normal person since everything has healed. Like a normal person, I should keep my blood pressure under 120. He cleared me for using Vyvanse (10-20mg a day) which I take most weekdays. Before surgery I didn’t have problems with high blood pressure.

Given that I don’t really have much of a risk profile anymore, do you think this is fine? I think it’s a comparatively low amount of blood pressure meds plus a low dose of LSD. If I’m cleared for Vyvanse, another vasoconstriction drug, I’d say I should be ok? To be clear, I won’t take Vyvanse and LSD together.

Would love some input here! Many thanks in advance.

How long have you been prescribed vyvanse?
From what you wrote it seems you won't be combining with the microdose which is wise.

Imo for all amphetamine-type medications their influence on the hormonal/adrenaline system is worth being aware of. They partly work by activating the stress system (HPA axis) which promotes hypervigilence/alertness, but stress hormones have other effects also (eg adrenaline → BP etc).

The effects are cumulative so if you've been prescribed it for 10+ years it's more relevant than say 1 or 3 years.

The Bisoprolol you're taking (blocks ß1-adrenergic receptor) may be helpful as LSD activates several adrenergic receptors. But since it's a microdose this is less of an issue.
 
Combining amphetamines with lysergamides is a recipe for general disaster imo, but importantly its a recipe for extremely high blood pressure (and generally uncomfortable experiences with very high body load), so definitely don't do that.

I don't think anyone here is actually qualified to give you proper HR here. Unless there is an actual doctor here.

However, in general, if you are cleared to take amphetamines then its probably safe to say that LSD, at any reasonable dose, won't strain your BP too much.

Just don't combine them. I have reached clinically absurd levels of blood pressure by combining meth + LSD + alcohol.
 
I was on Vyvanse between end of 2023 and mid 2025 then stopped for about 6 months after the surgery but have been taking a pretty low dose since then (10mg a day).

Microdosing LSD can cause mild, dose-dependent increases in blood pressure and heart rate. While generally temporary and well-tolerated by healthy individuals, these pressor responses mean that microdosing carries potential risks for people with uncontrolled hypertension or severe cardiovascular disease. Side effects like mild anxiety or elevated heart rate can further influence blood pressure fluctuations during dosing days.

Uncontrolled hypertension: Medical guidelines caution against psychedelic use—even at microdose levels—if a person suffers from severe, unstable, or untreated high blood pressure due to the acute risk of cardiovascular strain.

The side effects of microdosing, are minor and typical of psychedelics. They commonly involve transient, dose-dependent side effects such as increased blood pressure, a slight increase in tension and anxiety, and sometimes mild cognitive impairment. Other physical problems (gastrointestinal problems, headaches) may occur. The effects on mood and hallucinations in the case of microdosing are mild. These effects are minor and, in the absence of other conditions that could complicate patient condition (such as hypertension), are not dangerous.

What do they say about exercise intensity? Exercise naturally causes blood pressure (especially systolic) to spike, but a guidelines that says your systolic BP should stay under 110 may only pertain to measurements taken in accordance with standard guidelines in which you're supposed to be sitting at rest for some period of time beforehand.

A true microdose (under 10 mcg) might not be significant, but 30-40 mcg is not a microdose in my view. For most people that will be enough for above-threshold effects, likely including cardiovascular effects. The short of it is that psychedelics including LSD definitely raise systolic blood pressure well above "110" for most people for some time. I would also contradict some of what @mr peabody posted above and argue that these changes are not necessarily strongly dose dependent. In fact in some studies I've seen, higher doses of psychedelic can sometimes cause a smaller systolic BP rise than lower doses.

Another other potential concern is that LSD and psychedelics also increase heart rate somewhat, and the heart itself as well as the coronary arteries express 5-HT2a/b which psychedelics interact with in ways that aren't yet well understood. As such they may put some strain on the heart above and beyond what is caused by the BP increase.

Well, do you trust your doctor's judgment? He/she probably has access to way more information concerning your condition than any of us do. If anyone could make an informed decision, it's your doctor, but if you're here because you don't trust your doctor, then I'm not sure how we can help you. (In which case, why are you taking the Vyvanase?)

I'm not a doctor, and this is just a guess on my part. If you can tolerate reasonable doses of exercise (i.e. high intensity for a few minutes, medium intensity for 30 minutes, and low intensity for a few hours), then I think you'll be fine. If you are still following restriction on exercise intensity or have serious doubts about your performance, then you may be at risk. The risk is probably low given the strong safety record for LSD, but I won't say it's zero.

Well, do you trust your doctor's judgment? He/she probably has access to way more information concerning your condition than any of us do. If anyone could make an informed decision, it's your doctor, but if you're here because you don't trust your doctor, then I'm not sure how we can help you. I should have also specified that the 110 restriction no longer applies, it was for the first year which I passed back in July. My cardiologist said exercise is fine and didn’t put any restrictions, saying I can do so “at my own discretion.” I walk about 10,000 steps a day and can bike for more than an hour at this point.

Sounds like I’ll be fine. I was a bit more worried about the interactions between LSD and Bisoprolol and a very low dose of Ramipril. But this is helpful still, thank you!

Thanks for making that clear! I would not expect any problematic interactions with those drugs and a mini dose of LSD. If you want to be extra cautious in future sessions, you might titrate up gradually from your initial 30-40 ug dose (maybe try 60, then 90, then 135, etc.) to see how you react at each step, but most reasonably healthy people would probably be fine.
