The only time I took an opioid stronger than kratom (post surgery, for legitimate medical use) I got blochy red skin and spent like 4 hours vomiting anything I drank or ate including pure water.

Why?
 
red skin due to itchiness, vomiting due to nausea which are like the 2 most normal side effects even from the weakest opiates, try taking benadryl/hydroxizine whatever anti histamine for itchiness and nausea and see then if it persists it is an allergirc reaction.

Allergy meds are generally safe to take together with opioids. A pill with 50mg dimenhydrinate does the job for me. This is a very common substance found in allergy meds. Other types will work too but I just mentioned that one because it's one of the most common and it works well in reducing the histamine response that many opioids (and opiates especially) trigger.

Do you remember what opioid you were given? Your reaction sounds a bit on the extreme side of a general histamine response to me. It's possible that the dose was too high for you and/or that your body disagrees with that particular substance given. Morphine may be the OG, gold standard for pain relief in hospital, but it affects a fair number of people negatively (in the way you were affected). Synthetic (and some semi-synthetic) opioids are often easier for the body to handle (oxycodone, fentanyl, and others).

Thanks for the reply. I was probably given Ketamine probably 5 hours before I took the opioid, as I experienced a very similar comedown to the one ket users describe.

I know for a fact that I took hydromorphone. 1 mg dose. I have a BMI on the lower side of healthy.

Why?

Histamine release cause itching and stuff and sometimes hives. Opiates also trigger the chemo receptors thus causing nausea and vomiting.
