Opioids Oxyneo and cymbalta combo?

Hey,

Was wondering if it is safe to take Oxyneo 80 mg with my daily cymbalta dose of 30 mg. Any adverse effects or will it effect the oxy effect at all?

Well, according to this source, Citalopram inhibits Cytochrome P450. Oxycodone is a substrate of CYP450, however, it is metabolized by enzyme CYP3A4, wheras citalopram inhibits CYP450 and CYP2D6. Oxycodone IS effected weakly by CYP2D6, though according to this article, the increase in blood levels is pretty insignificant. However, when Oxycodone is taken in combination with a Cytechrome P450 CYP3A4 inhibitor, the levels of oxycodone increase significantly.

So you're probably safe taking the two medications, though I would definately look into it more thoroughly. From what I found, Citalopram works on CYP2D6, NOT CYP3A4. Fluoxetine and fluvoxamine will work on CYP3A4, as well as a number of other antidepressants/medications that you can again see here.

So your saying I may feel stronger effects from the oxycodone? - not so good with Pharmacokinetics so its hard for me to make sense of this table, but I see opiates and other anti-depressants or SNRI's are in the same column.
 
Just to be sure, you do have a high opioid tolerance, right?

80mg oxycodone is quite a dose for a lot of people.

Well, it sounds like there's a possibility it may increase the effects somewhat of the Oxycodone, but probably not enough that it would really be noticeable. It sounds like fluoxetine would possibly have more of a potentiating effect as it is CYP3A4 according to that table, though that's the first time I've heard that.

What that table doesn't say is how strong the listed drugs inhibitory effects are as well, which would also matter of course. I wouldn't encourage anyone to go pop a bunch of Prozac after consuming some oxycodone in an attempt to get higher..
