I have the same problem, and I think I also know why: when I was a little kid (like 4-5) I was in summer vacation with my parents and we were on the beach. This beach was quite far from the street, so in order to get the there was a 15-20 min walking in narrow paths in basically a desert land, with bushes, etc. Now while were walking back I errrrr really needing to take a leak, so since we were in the middle of nowhere my parents told me to just pee behind a bush. Well as soon as I started peeing someone popped up on the path, and I had to stop and put my pants back on. Then we waited, and I tried again, and again halfway through someone else showed up, forcing me to stop again. I think that caused some kind of trauma (I also was a pathologically shy kid, and still now I have some bad social anxiety issues, and if have that you know you are kind of always obsessed by other people's judgment) and after that I was no longer able to pee on the street, in a urinal, and even at home if my parents were in the bathroom.
Right now I still have the problem sometimes, but I kind of improved (I also worked on my shyness and social anxiety, and now I'm a functional person, or at least I look functional) over time.
I developed some tricks to deal with the problem, cause well I'm a pretty creative dude, so I'm gonna share them hoping they an help:
- when I was at home, and sometimes even with no one in the room I couldn't pee, I would open the faucet, just enough to have a tiny stream of water coming out, making that kind of trickling sound. I don't know if you can do that on that urine test, but if you can give it a try. The water sound was sometimes enough to take my mind away/relax me and let me pee.
- I would start counting, possibly in a foreign language I didn't know very well. At the beginning I was counting in English (Italian is my first language), then I became fluent in english and started counting in Spanish, then Polish, etc. Counting in a foreign language was often enough to require attention and take my mind away from the act of peeing, and I would be able to pee. If you don't know any foreign language even counting in your mother language can help, or you can try doing mathematical stuff.
- if you are really into something, try starting thinking about this thing and what you would do. Example: I was into computers at some point, and I would think speaking loud in my mind what kind of RAM I'd like to add, the motherboard settings, the graphic card, etc. Like I had to plan how to build up a computer from scratch.
It's really important you don't think about anything sexual, cause if you get aroused it's gonna be even harder to pee.
- usually if I can close the door that helps a lot, the maiproblem is when I cannot close the door. If I can close the door, I can then talk to myself saying stuff like: "the door is closed, it's totally locked, no one cam get in. No one can interrupt you. You can take all the time you need." etc. I basically try to feel comfortable. However if someone walks in even if my door is closed, sometimes just hearing that another person is close prevents me from peeing.
- if nothing above works, since this problem is about anxiety and you being too conscious about peeing, anything that can momentarily take completely your mind away from what you are doing could potentially work. I never tried with music, but maybe putting on earbuds and listen to your favorite song could work too. You just have to take your mind somewhere else for few seconds, and before you know you are peeing.
Let me know if you try my tricks and if anything works. I know how bad this stuff is (some times had to hold on with my bladder hurting like a motherfucker because there was no secluded place to pee, or separated toilet with lockable door)!
