The Pain Management Mega Thread v2.0

Hey guys

Hoping to draw on your experiences with pain meds
I have a pelvic upslip (my pelvis keeps twisting forward and up on one side) which causes inflammation of my sacroiliac joints, hip pain, back pain, headaches etc. at its worst, I'm bed bound. At its best, I'm normal but cannot engage in a lot of physical activity and need daily anti-inflammatories and heat packs.

It's been ongoing almost a year now, with a large gap of four months recently where I was prescription medication free. But sure enough it's happened again and I'm back where I started. I'm finding that the pain killers aren't strong enough for the pain, and that the diazepam is what's helping me. However knowing their addictiveness, I'm reluctant to ask my doctor for a regular prescription.

Anyone know of a muscle relaxant that is as effective but less addictive and less sedative that I could replace the diazepam with?

Thanks

Minxxxy

you could try (discuss with your doctor):

flexeril
skelaxin
soma
which are the 3 most popular/ effective.

Are you in a state with Medical Marijuana? If not you might ask about dronibinol. THC is very helpful for pain, inflammation, muscle relaxation, and sleep.

I'm having a serious issue right now that I need a 2nd opinion on. Okay, I got in an accident and herniated a disk in my neck, dislocated my shoulder, tore the radial ligaments in my right arm, shoved my collarbone out of place, and compressed the L5/L6 in my lower back. I completely lost the use of my right arm, and it went numb. I am also numb up my my shoulder, up my neck and into the right half of my face. The only thing I can feel other than numbness is extreme pain.

My doctor prescribed me 100mg of tramadol 3x a day, plus hydrocodone 5/500 2x a day, maximum. Needless to say, for my pain, this ain't doing shit. Not to mention when I realized that the hydrocodone was doing literally absolutely nothing, I found documentation that says that gabapentin 125mg doses lowered hydrocodone Cmax by 3%. 500mg of gabapentin lowered hydro Cmax by 21%. It also said that this effect is dose dependent. Well, I take 3200mg of gaba per day, and my doctor knows this. If that effect on hydro's Cmax is dose-dependent, then my dose of 800mg gaba 4x a day would render hydro practically ineffective.

I brought that documentation to my doctor, and told him I need help with the pain, and the hydro wasn't doing shit. He said he would be right back; that he needed to consult with his attending physician. He did not not even bring the documentation that I brought him with him. He came back and told me that I had build a tolerance to these pain medications and that he was going to discontinue the tramadol entirely and give me enough hydro 5/500s to take 1 pill a day. WHAT THE FUCKING FUCK YOU FUCKING IDIOT!!! I told him that was bullshit; look at my injury, it causes pain 24 hours a day, 7 days a week! 1 5/500 a day?! So I can have 2-3 hrs of shitty, non-effective pain relief per day!? I told him no, I want my tramadol; I've been on it at that dose for 5 years now for FMS, and that was before this horrible, fucked-up injury. And now you somehow think that my pain is less.

The fact is, I wasn't on enough pain meds to have my pain somewhat under control. I don't want to be a drooling mess, and I don't mind if I still feel some pain, but I wwould like to be able to have some semblance of a normal life. This isn't right of him, and I cannot afford another doctor. It's like he's labeling me a drug-seeker, when all I did was try to talk to him about getting on a different medication that might have this pain somewhat under control. My life is fucking miserable. I don't have any energy to do anything anymore. This is bullshit.

I don't know what to do and I need some 2nd opinions on what I should do. Thanks in advance, anybody.

get a new doctor. Check out my past posts in this thread about Lumbar Sympathetic Blocks, which is a depot shot of Midazolam and Fantanyl into your spine where the nerves that are fucked up connect, thereby blocking your bodies Sympathetic pain response.

Look for a doctor that deals with Regional Sympathetic Disorder and spine issues.

Twighlight - is this doctor you are seeing a pain specialist?
new doctor time for sure, i swear some doctors arent human at all.
i dont know what you can do about not being able to afford a new doc....
maybe if you post your country some others might be able to help?

im in a similar situation, too much pain, i end up buying drugs off the
street so i can get thru the tough days. not something i would recomend!

I need new meds, I seem to get only negative effects from morphine and no pain relief. I miss fentanyl. I wish they did it in extended release tablet form

I'm in America, Texas specifically. I'm enrolled in the Gold Card program in Houston. I had to because I lost my job over this injury. I am currently unemployable. I'm not sure exactly what I can do to rectify this situation, because no, my doctor is not a pain specialist. I don't know if I can respectfully confront him about this situation or not. The last thing I would want is for him to take my tramadol away as well...

A Gold Card is for people in Harris County (Houston); it's basically insurance for people who don't have insurance. I haven't been able to find a pain specialist within the GC program, although I met a guy who told me goes to one; he was script fent patches + 6 endocet a day. So there has to be one somewhere, his problems weren't even as severe as mine are. It's fucked up; I've always been good with doctors but I can't figure out how to talk to this one without being afraid he'll take away my pain meds entirely...

I have fibormyalgia ive had it for 3 years and is there any tips for dealing with my pain in the mean time before i go to a doctor? i get about like a 8-10 on the pain scale daily

From reading an above post, I must ask, do gaba-ergics counteract pain killers? I mean, GABA does inhibit all other neurotransmitters. Yes, this will help with nerve pain, but how exactly does this help with muscular pain? The painkiller I'm asking about in particular is tramadol. Also, would gabapentin affect morphine or stronger opioids?

I'm wondering if I need to come off of this drug, because I've actually been WANTING to get dry-needling done recently, which is very odd.

From what I read I think gabapentin reduces the effect of morphine. Not certain though. I had to come off gabapentin I couldn't cope with e side effects, and 3200mg a day didn't seem to do anything for my pain. Pregabalin was even worse side effects wise for me, maybe my brain doesn't like that class of drugs

^ How do you know it's FMS if you haven't been the doctor yet? Honest question.

I've had FMS for almost 5 yrs but I had a doctor officially diagnose me.

I have the pressure points and all that ive been to a doctor for it but he was a ass

GABAergics do in fact counter the effects of some painkillers, but not tramadol. I take tramadol; have been taking it for 4 years. Gabapentin/pregabalin both actually enhance the effect of tramadol. It mostly works by way of it's SNRI effects, then by the opioid action of 0-desmethyltramadol. I suppose in my experience, benzos can either decrease or increase the effect of tramadol...it sort of depends on if i'm suffering anxiety or extreme muscle tension due to the pain. But, I can definitely say that gabapentin + tramadol are an excellent combination.

And to answer your question about the morphine thing, you have the wrong opiate in mind. The documentation I found says that gabapentin lowers the Cmax of hydrocodone. The same documentation stated that morphine was not affected.

There are many different pain disorders that are triggered or have the same pressure points as FMS. Just thought you might like to know. See a good doctor who gives a fuck about you and your well-being...I know I don't have one right now.

So if I were to want to take vicodin, take the vicodin, then wait to take the gabapentin? also, when would I want to be able to re-dose the vicodin?

Oh my bad, I'm sure I read that it reduced morphine when taken together. Hmmmm need to find that info again purely because it will annoy me if I don't haha

I suppose that would be the way to do it, but as far as re-dosing on the hydrocodone, I'm not sure, and I've never read anywhere how long the gabapentin affects the Cmax of the hydro. Maybe you might consider not taking the gabapentin at all, unless, like me, you have to.

^ I happen to know morphine greatly increases the effects of gabapentin. It feels the other way round but alas no.

I didn't feel an increase in either effect. Saying that, it was a while ago. Just wish either ofthem would work for my pain, don't find morphine very good and I had to come off gabapentin due to side effects. Same with pregabalin, that was even worse for me.
