whats the deal with bupe docs?

ok, so a buddy of mine and myself are having a convo about how bupe is scripted.

he's saying it can ONLY be scripted by docs who have the special license. same with methadone.

methadone I know can be scripted by any doc that can write pain meds because its used as a pain med all the time.

but with bupe I can't figure it out. I know obviously the suboxone formulation is for opiate addiction maintenance as it has naloxone in it, so obviously that is one that you have to have the license for. but then I know there is subutex and temgesic, both of which are bupe only. temgesic is 0.2mg used for pain. subutex I think comes in the same dosages as suboxone. so, can subutex be scripted for pain by a regular pain doc or does that doctor have to have the special license and go to that class on addiction? I thought I heard if you write it for off-label use then technically you dont need the license because you're not writing it for addiction maintenance.

sorry this is so long, I was just really trying to make sure everything made some kind of sense.
 
true to write a script for suboxone/subutex they have to have a special license lol. their special license is a 5hr course online lol..im not sure y i just think they have to have some type of addiction education not all drs kno about addiction ive met quite a few that had no idea what suboxone even was and eventho they knew my history of heroin addiction kept handing me scripts for percs
 
Doctors haveto be licenced to prescribe buprenorphine for addiction but not for pain. There are formulations of bupe for pain like temegesic but I believe that suboxone ( i know for a fact ) and subutex ( i'm pretty sure, but not 100% ) are only fda approved for addiction treatment and not pain management.
 
Any Dr can write for sub for three days while a patient is waiting to get started in treatment, it's the law. Most Drs probably don't know about it. You can find the info at NAABT and print it out fo a Dr if you need to. Also Dr's can write sub for off label use, but that doesn't mean insurance companies will pay. They just approved a bup pain patch, no DEA number needed for that.
 
the only info on a bupe patch I can find is in the UK.

but I did read this and find it interesting.

so if you get that thing, you're pretty much saying you're done for GOOD!
 
The patch just came out in July, not suprising you can't find much info on it. I know it's made by one of the companies producing the generic subutex
 
I've replied to a thread on here about how to get the buprenorphine out of those patches. It's incredibly easy...
 
Its an 8 hr class. My dad is a bupe doc. He used to use temgesic all the time in ER. Just can't prescribe "high dosage formulation tabs" SUBOXONE, without the X in your first DEA letter. There just for addicts. Which is counter-productive. Buts that's are govt. Oh im so proud.
 
Of course this was back in the day when he used liquid cocaine to stop nose bleeds.
Told me one time he let a qtip sit little too much in the coke and gave it to a lady and said he waatched as her 1. Nose stopped bleeding of course. And 2. Eyes became very wide and started talking rapidly.

Im not a coke person but I wouldn't mind pharmaceutical grade liquid coke in the open membranes of the nose. Closer to IVing than snorting.

Yes...patch is approved for pain. I have never heard of sub being used off label. Even if possible...I doubt any docs want to be the first to do it. Never happens.

wait, he said theres no DEA number needed for a bupe patch? I call bullshit on that. you need a DEA number for any controlled substances. even if for only a 3 day script.

There has to be a DEA number written for the buprenorphine patch (temegesic). It is a Schedule III controlled substance. Also, any doctor can write a prescription for the patch or Buprenex (IV/IM) for pain. You need a special license to prescribe Suboxone or Subutex for opioid addiction. The prescribing dr. becomes an official "addiction specialist".
