trouble getting prescribed pain meds for real pain, please help

my twin sister is 22 and she's had chronic stress induced back pain since she was about 15 or 16. when she was 17 she did physical therapy for about a year, maybe less. that was out of state, and when she moved back home her back was better so she just stuck to stretches, light-moderate cardio, pot and heating pads.
but the pain this past year or two has been unbearable for her. pot does absolutely nothing for it now, too. unless its a lot of some really strong hash or edible but thats hard to come by and she can't go to work that stoned. she sees a therapist and psychiatrist to handle the stress but it sure isn't a cure for it.
over the summer at one point it got very bad, like 9 out of 10 bad. she had to do something about it, but it was memorial day weekend so she wouldn't be able to go to her GP for days. she's terrified of hospitals, so this girl she was seeing made her go to a small local urgent care with her. when the doctor was looking at her back it gave her a stabbing pain just when he touched it. he said it was unusual, but she didn't need tests and that it was probably stress. there's normally this "just stress" stigma, as if the fact that its stress induced means it doesnt hurt as much as injury induced. this is obnoxious because she has VERY high levels of anxiety causing hormones (they've done an experimental test on her, i think the test has been FDA approved since then) and is diagnosed with panic disorder, so the pain is very real and its unlike regular bad-day tenseness. anyway, this doc however was cool and sympathetic. he sent her home with an NSAID, tramadol, and one other thing she cant remember. none of these really worked. she was less debilitated, but debilitated nonetheless. it took like a miniscule amount of the edge off. tramadol worked a little more than the others, it helped her sleep which was hard to do with the pain. but i believe its also an SSRI and she gets bad reactions to those.
the second memorial day was over, she set up an appointment with her GP who felt her back and said she was stiff from stress and didn't need any tests done. she, desperate for anything that even worked a tiny bit, asked him if he could refill anything that they gave her at urgent care and told him that tramadol worked the best (maybe that was a mistake?) he too-confidently and firmly told her she didnt need any of that and referred her to a specialist.
well, the specialist was booked ridiculously and she couldn't wait, so her mom referred her to this chiropractor. she had seen one before who she hated, but this ones great. only takes 15 minutes, cracks her neck and back, really takes the edge off. she also gets weekly massages.
but she can't be going there all the time, and she's still usually in pain and its effecting the amount of things she can get done in a day. theres very little discussion when she see the back doctor, he's like almost intimidatingly quiet. so she always gets too nervous to ask him about pain meds. she doesn't even know if he's the kind of doc that prescribes them.
shes at a point where she can't even do her job and takes days off from it. (she does pretty physical work)

she has a friend whose coworker is a major pill popper, so she has been getting norco 10's through her and sometimes her dad gives her one or two soma 350's. these work tremendously well compared to the urgent care meds when combined (in small doses, she's careful.) she has used pain pills in the past and they've always been a godsend, but they've never been prescribed.

what sucks now is that her dad no longer gets prescribed Somas, and her friend with access to norco lives outside of town and is becoming very increasingly unreliable. she hasn't been able to be reached in maybe a week. and when she pulls through she gives my twin much less than what she asked for. (doesn't charge for the missing ones though.)

so my twin was wondering what she could do for all of this and how to get prescribed an opiate and or soma. (soma doesn't really work on its own though) she doesn't think it would do any good to go back to her GP, but should she ask the chiropractor? get an MRI? find another doctor? what should she say with out sounding like a drug seeker? what should she do if he doesn't give her anything or anything that works? she really wan'ts opiates for as needed day time use, because unlike soma, benzos and muscle relaxers, they dont make her drowsy.

any advice?

I also have chronic back pain, so I know how debilitating it can be. Your sister needs to go see a Pain Management doctor. And just be warned, that it's even difficult to find a good PM doc that will prescribe opiates, so don't be discouraged if it requires seeing multiple doctors until she finds a good one. Look up the pain management doctors that are on your insurance and in your area, then go searching online for reviews until you find one that sounds compassionate. She should demand an MRI, as if there is an actual physical explanation for the pain, the doctor will be FAR more likely to prescribe meds. My bad MRI is the best ammunition I have when I've had to see a new doc, and I imagine it would be near impossible to get opiates for very long without it. An MRI and searching for a good PM doc should be her next steps. Good luck!

Even if the specialist is booked, your sister will have to make an appointment and wait to see that doctor if she wants serious help. We can't advise you what to say to a doctor in order to get narcotics prescribed so I have to close this.
