Bupe Suboxone taper/detox chart

I'm looking for a Suboxone taper/detox chart I remember receiving from my past prescribing doctor. I've done a few searches but wasn't able to locate it or one..
 
A typical heroin detox in Texas was:
1st day-8mg
2nd day-12mg
3rd day-16mg
4th day-12mg
5th day-8mg
6th day-4mg
7th day-2mg
All were sublingual ROA.
This was the easiest heroin detox I've ever done and worked close to ten times just fine. The only problems I had were when I chose to use H again during the detox which destroyed the effectiveness after two days of backsliding.

It is possible to detox on much less, at least with subutex it is. I have seen an easy detox that was:
1st day-2mg
2nd day-4mg
3rd day-2mg
4th thru 7th days 2mg
All doses were IV ROA and subutex not suboxone.

It should be remembered that the best results are achieved by waiting as long as possible(24hrs after last opiate dose is usually adaquate, the more dope sick the better) into withdrawal symptoms before beginning the buprenorphine as otherwise one can experience precipitated withdrawal if treatment starts too soon.

These numbers are only good for short acting opioids. Do not use this as a guide for methadone detox!

It depends on what opioid you're dependent on and how much you take of said opioid for one.

As for the rest, unless you have a very heavy habit, please do not do what the person above me said, those doses are way too high, it's a waste really.

2-4mg should be enough to hold most habits, 8mg MAX. And by that, I mean first dose, then maybe again the 2nd day, the go down 1mg every day
until you're down to 2mg, then do that a few days, then 1mg a few days, then 0.5mg a few days, then 0.25mg a few days, then skip a day, then 0.25, then skip a day
then 0.1mg for 2 days, then do that every other day for maybe a week, and then try to stop entirely. That should work very well and you shouldn't have any real withdrawals that way unless you've been using a long time.

Good advice here. Don't I repeat don't pull initial dose amounts out of your ass. The best way to induct is be in moderate withdrawals. then take 2mg. wait an hour, this is important because Subs take awhile to reach effectiveness. Then if you still feel like shit take another 1mg and repeat process. The idea is to start at lowest dose. Don't just say "I think 4mg or 6mg to start" big mistake. Once you get to dose where most withdrawals have been eliminated that is your starting dose. Stay on that dose for a few days, you can split that dose at first (morn-night). After 4-5 days start to reduce, the amount really depends on many things so you have to decide what amount. Some people have different results, some need to be clean by a certain date. Try to eliminate the 2 a day doses. Once you get down to less than .5mg start to skip a day, dose and skip 2 days try to get to 4 days. If you fail at first try again, dose skip a day...etc. Once you get to day 4 then stop and use OTC meds. Also people advised you on exercise, it is hugely important. All it really takes is daily walks while tapering, it will reduce any withdrawal effects. Try to walk in the evening as it makes getting to sleep a little easier. One more thing, if you are done with opiates then make other changes while tapering, lose the drug buddies, phone number, reconnect with non users, I'm sure you know what I mean. Most fail here and just take Subs and nothing else, that leads to such a high relapse rate it is almost worthless to even start the Subs in the first place. Start smallest amount - Taper - exercise - make changes in life - your on your way to a new life with as little discomfort as one can expect from freeing yourself from opiates. It takes work, there is no free ride out of opiates but this is the least painful. Best of Luck
