new to PM docter. but he's got me concerned.

not sure if this is appropriate place this would go, if not mod pls move......Anyways. I went to my first appt and was given 45 oxy 10/325s for 2 weeks. before going to pain management I was on suboxone for about a year from a different car accident. I've been off it now for a month or so but I still see my psych doc an he gives me so as,addys,klonopin(been on for years same as addys during high school an now college). I was involved in an accident that resulted in several spine problems that give me severe problems...... anyways. I went to call him today to let him know I didn't think the oxy 10s were strong enoug(he told me I could call him if it was too weak) an he started going on a tangent about how many controlled substances I'm on. klonopin started young as I had seizures and adder all for ADHD. the slams were recent for severe back spasms. I said am I kicked out? or am I still able to come to next appt? he said I can still come. But, the 10/325 feels like aspirin after being on subs so long. I'm not drug seeking. I'm just looking for relief without trying to sound like an addict(which it may look as I was given 90 subs a month as psych doc told me they help for pain) what can I say or do so I can get proper care? any advice anyone? also I am 24 as well if that can help anyone's answer be more correct and also have Had a recent MRI done.
 
Hello, whilst I do sympathies with your situation, it's notoriously against the user guidelines of BL to advise members on how to source medication(s) from Physicians/Consultants. Whether legitimately or otherwise, prescriptions or illicts...

My only advice would be to see what you feel you can cut back on and sit down with your Consultants and have an open and honest conversatio.
 
I'm not asking for sourcing. I'm just asking opinions if I should offer to give up soma even though it really helps with my spine spasms or adderall for my ADHD would help me with getting better treatment. and making sure its legal to see a psych doc and a PM doc

We're basically not allowed to offer any advise on what to tell your doc in connection with medication. Sorry. I can tell you it is certainly legal to see a psych doc and PM doc at the same time. You're just not supposed to get narcotic pain meds from more than one doc.
 
Its an uphill battle I know.. Was on methadone 2y. Then I got chronic pancreatitis. They maxed me out on tramadol 400mg dly but it didnt do anything for the pain cause the methadone obviously gave me a tollerance not to mention the meds that I was put on before mmt.. My best advice is be open and honest, tell the doc your history, do some research on equianalgesic properties of opioids but keep at it and dont be scared to go for a second opinion. And honesty is best. If your in real pain nothing will stand in your way. .

It sounds like you're approaching your PM doc and your visits with the wrong attitude, sort of like you're already guilty of something. Wheeling and dealing (offering to give up something to be able to get something else) with PM docs is totally the wrong way to go about things. Generally, good PM docs "do" want to know if what they're prescribing is doing any good. Secondly, yes they will call you on things if doses are starting to get out of hand because there is clearly a theory in play of "increasing dosages with diminishing results". In other words, if the doc were to double your dose, you clearly would not get double the pain relieving result, and so on and so on.

This is why the communication between doc and patient is so important. You need to be on the lowest most effective dose possible and that can only be achieved with good and proper communication. This way you reduce the chance of side-effects being a problem and hopefully you can maximize it's pain relieving qualities, realizing of course, you'll never hit zero on the pain scale. On another note, and I've never been on them so I may have this all wrong, but don't subs inhibit narcotics one way or another. In other words, if taking subs regularly one would not feel much of anything taking a few oxys. Is that right? This may explain why the meds they're giving you don't seem to be working. If I have this wrong, I apologize.

Also, it's generally not a great idea to have two or more docs prescribing meds like this. It's best to funnel all the scripts through one doc so they have a good handle on what all you're taking and what result you should expect. Hopefully, you get what you're looking for and if I have some of my facts wrong, I am sorry but I'm just trying to help out here.
 
Couple questions;
1. Was the subutex treating your pain better than the oxy 10's are currently?

2. Why did you get off the subutex ? Was it given as a pain med (prescribed) or for addiction?
Here's the main problem as I see it, you're new to your PM doc AND you come with existing scripts for the most notorious abusable drugs. It doesn't matter if you abuse them or not, it's what it "looks like" and TBH it doesn't look great.
I would do a pain journal, write down your dosing schedule for each day and put a remark as to how the pain is. Bring it with you. If no contract was initiated by him, you could offer that as a good faith gesture, to show you're ready to have a pill count or test dirty at any time to show you're using your needs as prescribed and not diverting.
Appearing too knowledgeable about opiates or using slang is a bad idea. Don't talk about the meds, talk about your pain.band obviously, go in relaxed , clean, presentable, confident (not desperate even if you are desperate to treat the pain)
