If you live in a state where medical marijuana is legal and you want to exercise that option as a patient, the best thing to do is find a cannabis specialist (do a Google search or look on the back pages of your local free paper) who can best advise you as a medical doctor as to how marijuana can best enrich your life.
It really isn't necessary to lie or exaggerate symptoms. Just be on time for your appointment and dressed neatly, be honest with your doctor, have copies of medical records if you can get them (most doctors don't ask) and once you have the right to use medical marijuana, don't put it in jeopardy by dealing or driving impaired.
The state ID card at least in California is not mandatory. Your doctor's note suffices under state law. Some dispensaries will issue you a dispensary ID card with your photo and a shrunken version of your physician's recommendation on the back so that you don't have to carry your papers on you (you should have a copy regardless, at all times).
