Benzos Lorazepam - Ideal dosage for guaranteed sleep?

Hi all,

Apologies if this is a stupid question as I appreciate substances effect everyone differently and tolerance is also relevant.

As this is my first ever post, I suppose I should start off by saying that I’m not (nor have I even been) a recreational drugs user. However due to a previous spinal injury leading to cauda equina syndrome which required two major surgeries on my lumbar spine to repair I had cause to be prescribed diazepam and various opioid pain killers ultimately building up to a 100mcg fentanyl patch. Diazepam dosage was fairly low at around 2mg 2-3 times daily however it was fairly long term ie: 6-12 months.

I am now free of any opioids or any other pain killers and no longer use diazepam.

However, I suffer from pretty extreme restless legs syndrome which has proved resistant to treatment from dopamine agonists such as ropinirole etc as well as pregablin and gabapentin. My neurologist currently has me on 1-2mg of Lorazepam as needed with 2.5ml oramorph alongside it although I don’t use the oramorph as I will never forget how I’ll I felt when I stopped fentanyl. The lorazepam doesn’t seem to be having much, if any effect. I tend to play it carefully with medicines and don’t generally change dosage without speaking with my doctor however I’ve spoken to my GP twice and they are reluctant to alter the prescription and I’m not in with my neurologist for another two months. And this week the RLS has been killer to the point where it’s driving the mrs just as mad as me as I’m keeping her awake aswell.

I’ve tried up to 4mg and this didn’t have much of an effect either. Am I just immune to the stuff, am I missing something or is my dose just too low? If it helps I’m a 35yo male 181cm, 80kg although I’ve no idea if this effects dose calculation. I haven’t and don’t intend to use lorazepam on a daily basis. I tend to use it once or twice a month when my RLS hits its worst. This has been a bad week for it.

I would be grateful if anyone who has experienced anything similar could advise me? Just to reiterate, I’m not looking to get high - I just want a good nights sleep but would like to be sensible and make sure I still wake up in the morning, if you catch my drift.

Any advice would be appreciated.

Lorazepam isn't a particularly hypnotic benzo. You need to take more than usual, of get diazepam, Xanax or nitrazepam, of another more hypnotic benzo.
 
A fair amount of people seem to not respond to oral lorazepam.

Clonazepam is superior in the treatment of RLS and may bring you the rest you're looking for.

Hi, thanks for this. It’s really useful to know that as I don’t know a great deal about pharmaceuticals. Perhaps Ironically my Neurologist had suggested clonazepam in the first place in the letter to my GP but for some odd reason my GP went with Lorazepam because he said it’s shorter acting and less likely to make me drowsy the following day. I’ll definitely raise it with the specialist when I see him next - in fairness he is very ameniable and happy to try whatever might help, it’s trying to get my GP to listen to me in between the appointments with the specialist that’s proving difficult.

Yeah it's super short acting. It's great for anxiety but not the best for sleep. It's not uncommon not to feel anything from medications like this if you aren't anxious at the time. It won't help you to take any more than 4mg, no use in increasing the dosage. :)

Hi, thanks for the reply. This is really useful to know as at-least I know I’m not imagining it. I was honestly starting to wonder if my pharmacist hadn’t just given me placebos or something. I’ll have another chat with GP tomorrow and see if I can get switched on to clonazepam or anything a little more sedating. I imagine he will want to wait a while as I’ve just been prescribed 60 lorazepam tabs but tbh he can have them back for all the food they’re doing. 🤷🏻‍♂️
 
Clonazepam at 2mg usually works, longer lasting than lorazepam
 
Lorazepam 1mg for sleep but you'll eventually build up a tolerance. Excellent benzodiazepine probably my favorite benzo.
 
Clonazepam 2mg would be far superior to Lorazepam for RLS. In fact according to Wiki Clonazepam is indicated for RLS!

Cheers. GP wrote me a script for x60 clonazepam tablets today so I’ll give them a try. You reckon 2mg will be enough to help me fall asleep?

Hhahaha :). Clonazepam is a very potent benzo so be careful with it and its got a very long half life too, it ranges from 20-60 hours lol. Try not to take 2mg, try 1mg (halve the pill) first and see how u go. Only take them as required otherwise you'll have a hefty benzo dependence on ur hands!!

yeah UK based - London as it happens. So t worry - I only use them about 3 nights a month when I get the RLS. I get such a hangover like feeling off then in the morning that I wouldn’t want to do it anymore.

I need the 2mg as I was on quite a large amount of diazepam every day for over a year due to my spine injury so 1mg doesn’t quite do it. I might try 1.5 though.

How on earth did u get a prescription of Clonazepam for 60 pills in the UK? GP's in the UK religiously won't prescribe any benzos at all no matter what and I'm sure a lot of UK users on here will back me up on this! Did u get them off ur GP or a Psychiatrist? Are they on repeat? I'd be very lucky to get a mere few Zopiclones off my GP if I had a relapse on my insomnia!

Fair question - they’re not on repeat so I have to go in each time I want them but I’ve never had an issue. The prescription was set by a consultant neurologist for severe primary RLS (although personally I think the RLS might be secondary to undiagnosed but definite ADHD) but maintained by my GP. How I got them? It’s kind of simple I guess but a combination of factors. - I’ve been with my GP since I was 7yrs old. I have a highly sensitive job in which I am drugs tested quarterly (hair strand). I have never used a recreational drug in my life - not even weed. I have tried every possible non pharmaceutical recourse to the RLS including running 10k at 3am, attending support groups, years of psychotherapy etc and when I did come to medications I really tried my best with the less controversial meds first. When I do use benzos for as needed use, they last me months at a time and I’m not regularly appearing asking for mI suppose the easiest way to put it is that my GP (and my specialist) have a great deal of trust in me and I have never given them a reason not to (not would I).

that’s not to say I’m making any judgements on anyone else, all I can say is that I have always been 100% honest with my doctors (and my employer) and never tried to have them over. if I feel I need to increase a dose I don’t just do it, I go in and speak to them first and explain my situation and always respect their advice. Possibly what helps is that before I was 32 (now 35) i had never been prescribed a medicine in my life and had done everything possible to manage my ADHD, RLS and even PTSD Through excercise and therapy and only went to GP when it became too much to cope with.

Idk. Took 60 mg olanzapine. Not asleep yet.

Not sure what Olanzapine is? I’m told by my neurologist Temezepam is the best benzo for sleep but he opted not to put me on it because I was on 2mg Clonazepam and the conversion worked out something stupid like 80mg Temazepam which seemed somewhat excessive for me as while im not entire benzo naive ( thanks to my year or so on daily 20mg Diazepam, occasionally IV) i felt like it might put in a sleep I wouldn’t wake up from.
 
As others have said, Lorazepam isn't very good for sleep.
Even when I saw benzo-naive and prescribed 7.5mg/day for severe anxiety it never made me sleepy at all.
If you want a benzo for sleep, any of these would be much better options:
Nitrazepam
Temazepam
Triazolam
Diazepam (not really a sleeping benzo but 20mg always worked well for me)
Flunitrazepam
Alprazolam

The reason the Olazapine isn't working is because you took way too much. Atypical antipsychotics are less sedating the more you take. 5-10mg would make you sleep. Much more than that won't.

How did you take IV diazepam? I've tried that before but it just kinda turned into a mush on the spoon that was no way gonna get into a needle.
 
I'd advise against using antipsychotics due to their serious side effects if taken long term. They can cause conditions like Parkinsons, Tardive Dyskinesia, and Neuroleptic Malignant Syndrome. Benzos are safer although they do have an addiction profile.. addiction can be cured. Parkinsons, Tardive Dyskinesia, and Neuroleptic Malignant Syndrome can't.

Anticonvulsant Mood Stabilizers r much safer (except Lithium) than antipsychotics.

How did you take IV diazepam? I've tried that before but it just kinda turned into a mush on the spoon that was no way gonna get into a needle.

i was a hospital in patient after neurosurgery for cauda equina. It was a specific IV solution administered by a nurse - not something I tried myself from a tablet. Sorry if that confused
