Opioids Fentanyl tolerance??

Today I was prescribed Fentanyl patches, 50mcg. I have a tolerance for opiates. I can usually handle 3.5 tab 10's every 4 hours or 2 perc 10's every 4 hours. I have also used Roxy 30's in the past. For some reason they didn't work as well as the lortab. I would have to take a whole one and then a quarter of another one, just for it to work. I have never used Fent before, and I'm just curious if the dose is too much or not considering I have a tolerance. How long does it take to kick in? Is it the same type of feeling you get from just regular pain pills?
 
When you say you have a tolerance, are you physically dependent? I mean, have you been taking opioids every day for 6 months or more? Because patches that strong are only meant for people who have a real tolerance built up and need 24-hour pain management. If I had to estimate, without finding a conversion chart, I'd say wearing that patch is equal to ~120mg of hydro/day, if not more.

If you were legitimately prescribed them I'd say wear it, your doctor gave it to you for a reason. Just be careful. Fentanyl is very powerful and the line between effective, FUCKED UP, and overdose is pretty thin, especially if you mix substances.

When I was Rx'd them it took about 4 hours after putting on the first patch before I felt anything, and at least 8 before it plateaued.
 
I have rheumatoid arthritis. I have been prescribed pain meds for almost 4 years now. He gave me 2 50mcg patches to use and another script for the 75mcg patch. He told me to use the 2 50's first. I just took the first 50 off. It seemed to make my arm a little warm for the first few hours and only took about 45 min to start working. I didnt get a buzz or anything off it but that isn't why I put the patch on. It seemed to work for the first 2 days but not the third day. I have always had a high tolerance for medication. I didnt take anything else with it, as im not chasing a high. I am curious if I will see a huge difference from the 50 to the 75. Like I said, it seemed to kick my pain for the first 2days. Hopefully the others will work for thr full 3 days.
 
Do you mean 100mg of oxycodone per day? Because I have never taken that much at one time. Within a day, yes. But never at on time.
 
Yes, per day. The way the patch works is that your skin literally just absorbs it, but at a constant rate. It's supposed to work for 72 hours but many people find it less effective after 48. The 75mcg patch will be 50% stronger. If you needed a bit more with the 50mcg patch you should be ok with the 75. If it starts making you sick just take it off.
 
Considering that your tolerance to hydrocodone or oxycodone weren't too high as far a prescribed doses for chronic pain management are concerned, I'm wondering if it's possible for you to explore the option of using the extended release version of those medications rather than fentanyl, especially since those first two meds worked well for you, but the fentanyl which is more dangerous didn't work that well for you.

The extended release version of hydrocodone came out pretty recently under the brand same 'zohydro' so that's something to discuss with your doctor. It may be better than the extended release version of oxycodone that came out since the mechanisms which were made to prevent abuse of the medication also made it less effective in terms of pain management. Of course everybody is different though, and it may actually work for you, so discuss it with your doctor and go from there.
 
it sounds like your current tolerance is nothing for fentanyl so please please be very careful. this is a much stronger drug.
