In your survey there is no indication of who you are, your institution, what you are studying - any kind of introduction to you. Nor is there any discussion of the ethics of your project - what you intend to do with the data, how you might feed information back to participants. I understand being a university assignment you may not have gone through ethics approval but it's good to learn about these things anyway. You can click on some of the university surveys advertised here for some idea of how to construct this sort of information in future.
